IP completed loading railcar & advised trackmobile operator they were ready to move car while trackmobile Operator was pressuring up air system IP decided to go back on top of the car without notifying trackmobile Op. IP was strapped into fall arrest system which caused them to be drug across the top of moving cars. IP suffered a fracture to a vertebrae and bruising to torso.

Showing all 22Employee was in the process of connecting a car puller cable to a rail car. The employee did not wait for the railcar to come to a stop before attempting to place the hook on the rail car. The hook on the car puller pinned the employees hand in between the car puller cable and the railcar. This resulted in the employee breaking a bone in his left hand.
Employee was operating a scraper when she felt hit a bump and felt a sharp pain in her lower back. The employee was seen by a Doctor and the results of an MRI revealed that a compression fracture occurred in the L1 vertebrae.
Contractor hauling and stockpiling crude ore @ facility. Contractor backed truck up to stockpile. When he attempted to dump, the cab of the haul truck came off of the ground. The cab turned to the right hand side and when it came down, haul truck operator's arm broke right side window causing laceration to contractor employee right forearm.
Employee was being trained on the water truck, the trainer released him to drive on his own. He was descending a pit ramp when his brakes got hot. The truck gained speed and went across the pit floor through some rough terrain & stopped on a bench. He indicated neck pain so was taken to be medically examined by a physician. First aid only, he was given restricted duty for 1 week.
The employee put all of his PPE on including the proper fall protection. The employee failed to notify the operators whom normally work in the area of his intentions. The employee began changing the light on the railcars when he felt the cars begin to move. The employee lost his footing and hit his head. The employee received 4 staples in the back of his head.
Employee was sweeping the floor inside the mill building when another employee approached his area with a forklift while carrying a pallet of packaged product. The employee driving the forklift set the ~4000# pallet down on the left foot of the employee who was sweeping, causing a crush injury.
A contract company was hauling crude ore into the plant and the driver rolled his truck. The only injury was a bump on the head.
While stacking 100# bags of barite on pallets employee's left hand got caught between moving rubber conveyor belt and non moving roller belt. Employee was wearing gloves and the emergency stop button did work stopping the belt. Hospital said employee strained muscles in hand & arm.
Employee was operating Volvo L180 loader. He was preparing to dump the 68th load into 17 belt hopper. Just before he tilted the bucket to dump into the hopper the shaft nut assembly on the dump cylinder came apart and the bucket rolled back. The material came through the front windshield into the cab. The force pushed the steering wheel down on top of both legs above knee.
Maintenance crew was repairing bucket elevator when injured employee called out for another employee to STOP advancing the elevator. The instruction was misunderstood and the elevator was advanced instead. This action caused the laceration to the left thumb area.
Ee was attempting to dislodge by hand a 50# bag of product that had gotten stuck at the corner of the palletizer belt system: he caught his glove between the belt tail pulley and thr guide that keeps the bags on the belt, lacerating the middle finger of his left hand.
EE was operating haul truck when it slid off the side of the roadway, tipping on its left side.
Employee was driving a haul truck when he came upon an icy corner. as he applied the brakes his truck jack-knife.
Ee was unplugging spout on glue box attached to semi-automatic palletizer while belt & roller assembly under box was still running. A product bag came through while he had his hand under box & caught his left hand & wrist between bag & roller on glue box. Ee finished his shift & sought medical treatment that evening for swelling of left wrist.
EMPLOYEE WAS ATTEMPTING TO OBTAIN A PRODUCT SAMPLE FROM A SAMPLE PORT (2" PIPE EXTENDING FROM SCREW CONVEYOR AT #3 TANK). HE FOUND THE PIPE WAS PLUGGED, THEREFORE HE ATTEMPTED TO CLEAN OUT THE END OF IT BY INSERTING HIS RIGHT INDEX FINGER INTO THE PIPE. THE SCREW CAUGHT HIS FINGER, CAUSING A DEEP LACERATION AND FRACTURE.
OPERATOR OF THE WATER TRUCK WORKING ON THE HAUL ROAD ATTEMPTED TO GO AROUND A CORNER WHEN WATER INT HE TRAILER SHIFTED, CAUSING THE TRUCK TO ROLL PINNING THE DRIVER IN THE CAB. THIS ACCIDENT R EQUIRED MEDICAL ATTENTION FOR HIS INJURIES.
EE WAS OPERATING A 637 CAT SCRAPER WHEN HE ENCOUNTERED ROUGH TERAIN AND JARRED HIS BACK.
OPERATOR WAS RUNNING CAT 988F LOADER AND ENTERED THE STOCKPILE. GROUND WAS FROZEN AND EJECTED OPERATOR INTOWINDSHIELD. NO SEAT BELT WAS WORN, WHICH WAS IN GOOD, OPERATING ORDER.
EE WAS HANDLING 50LBS BAGS OF PRODUCT FROM THE PACKING MACHINE TO A WOODEN PALLET. WHEN REACHING TO DISLODGE A BAG THAT HAD STOPPED BETWEEN THE END OF THE OCNVEYOR AND THE ROLLERS, HIS GLOVE G OT CAUGHT BY THE CONVEYOR WHICH PARTIALLY DREW THE FINGERS ON HIS LEFT HAND DOWN BETWEEN THE CONVEYOR AND THE ROLLER, CAUSING A LACERATION AND CRUSH INJURY TO TWO OF HIS FINGERS.
EMPLOYEE WAS PALLETIZING 100# BAGS OF PRODUCT WITH A NEWLY-HIRED EMPLOYEE. ONE BAG CAME OFF THE BELT OUT OF POSITION. WHEN NEW EE ATTEMPTED TO CATCH IT, INJURED ALSO TRIED TO CATCH IT. OUT OF POSITION MOVEMENT CAUSED MUSCLE STRAIN IN HIS BACK.
EE WAS MOVING CARS WITH 9TM TRACKMOBILE. THE CAR HE WAS MOVING COUPLED INTO ANOTHER CAR AND HIS HAND WAS RESTING ON THE WINDOW SILL. THE WINDOW SILL HAD A CRACK IN IT AND HE CUT HIS HAND ON TH E SHARP METAL.
