Contractor hauling and stockpiling crude ore @ facility. Contractor backed truck up to stockpile. When he attempted to dump, the cab of the haul truck came off of the ground. The cab turned to the right hand side and when it came down, haul truck operator's arm broke right side window causing laceration to contractor employee right forearm.

EE on ladder pulling air tubing through conduit. EE's hand slipped off of tubing as he was pulling. Employee grabbed top of ladder, when he did, the momentum caused him and the ladder to fall backwards. EE landed on tail bone causing injury to pelvis area. EE was not tied off. EE did not have ladder in fully opened/locked position. EE did not have ladder secured.

Employee was inspecting truck to load. Employee lowered inspection stairs to top of trailer. Inspection stairs got hung up on trailer hatch. Employee tried to use pneumatic system to raise stairs. Stairs remained lodged. Employee attempted to dislodge by lifting on cage. When lifted, cage became dislodged and upper railing struck employee in chin causing laceration.

Employee was trying to move a belt manually and caught his finger between the belt and sheave.
Employee was transporting 2 sheets of grating with a forklift. There was a piece of loose grating between the 2 sheets that was bouncing around. The employee stopped the forklift. He went to the front of the lift to attempt to remove the loose piece of grating. The 2 sheets shifted and smashed the employees finger, tearing the fingernail.

EMPLOYEE WAS NEAR A CO-WORKER THAT WAS GRINDING METAL AND A SMALL PARTICLE GOT INTO HIS EYE.
Employee was struck by a chain hoist when it came unhooked from the point of hoisting pressure. The tank leg was under tension with a chain and hoist. The leg jolted loose and sent the cain of the hoist flying and struck employee's back.
Employee was using a pry bar to lift and align a transition chute for installation from screw conveyor to an inlet for the silo when the pry bar dislodged while applying pressure to align the unit. Employee slipped and fell backwards onto a sharp metal object.
