A contract company was hauling crude ore into the plant and the driver rolled his truck. The only injury was a bump on the head.

Employee was trying to change a belt on an electric motor pulley by himself and his hands got stuck between the belt and pulley. Second degree burn and contused left hand and wrist.

Maintenance crew was repairing bucket elevator when injured employee called out for another employee to STOP advancing the elevator. The instruction was misunderstood and the elevator was advanced instead. This action caused the laceration to the left thumb area.

Employee was removing bolts on a loader bucket when bolt spun off cutting though his gloves and lacerated left hand.

Employee was pulling conduit to re-connect the compressors. Musculskeletal pain.

Employee was trying to remove a rattle snake in truck loading area. Snake bit employee on finger of right hand.

EE WAS WORKING ON A PIPE WHEN IT ROLLED OFF OF THE WORKBENCH, FALLING ON HIS RIGHT FOOT.

EE WAS HOLDING A MOTOR IN PLACE TO BE BOLTED.

EE WAS REPLACING THE TANK INSPECTION DOOR, WHEN THE DOOR SLIPPED PINNING HIS THUMB BETWEEN DOOR AND TANK.

EMPLOYEE WAS CLIMBING STAIRS TO A MILL TO SERVICE IT WHEN HIS FOOT SLIPPED, CAUSING HIM TO BRUISE HIS RIGHT KNEE.

EE STEPPED OVER THE SCREW TO THE DUST COLLECTOR AND SLIPPED, SCRAPING HIS LEFT LEG.
