Employee was leaning into mantrip giving out ear plugs unaware of his right foot was half way under mantrip, mantrip moved catching right foot under mantrip and twisting foot. Employee received a fracture to his right ankle. Employee has returned to work with a full release.

On 8/14/2013 employee was traveling on rubber tire buggy when he ran over a rock buggy bounce causing head to hit roof top, employee continue to work received a full release 8/27/2013.

On 4/30/2013 employee riding in mantrip strained neck when mantrip struck parked mantrip. Employee became restricted duty on 7/23/2013 due to this injury.

Employee was at the end of Lo Lo structure when he placed his left hand on back of carrier dolly when rock came across dolly striking left hand fracturing his left ring finger. Employee has been placed on restricted duty.

The employee got off a fork and got on another forklift and backed into the forks of the other machine, the fork struck his left leg. He went to the doctor and had 12 sutures placed in the cut he received. He returned to work the next scheduled shift. Full duty.

Employee was operating mantrip, when mantrip spun out on hill. Mantrip started rolling backwards, mantrip hit 20 ton diesel motor that was on track. Employee was placed on light duty with bruised left hip.

Employee was standing at roof bolting machine when scoop backed out of right break into #4 entry. Bucket of scoop caught corner of rib and broke rib loose causing rib to roll onto EE and roof bolting machine. Employee received a fractured collar bone and 6 left ribs were fractured, employee became lost time on 12-10-09.

Employee was operating scoop with left hand outside of deck, left hand got caught between bumper stop and deck, causing injury to his left index finger. Employee became lost time on 4/08/09, left index finger tip removed due to this injury on 4/7/09.

Employee was taking scoop back to section when he ran into some belt structure. Structure came into the deck of scoop pinning him inside of the deck bruising his right side ribs.

Employee was dusting in neutral entry, scoop hung into lifeline pulling hanger, hanger broke loose hitting employee on left side of head, sutures required.

Mantrip came to a stop and EE struck his head. Sutures were required. He returned to his regular job on his next scheduled shift.

Employee was riding in a mantrip when the mantrip de-railed; employee complaining of back pain. He was placed on restricted duty 4 days.

Employee alleged that while driving a mantrip he received whiplash. This injury became reportable on 9-20-07

While operating a scoop he ran over a belt stand, it flew up and hit him in back of the head. Small cut on back of head, restricted duty for 2 days.

While hanging a curtain the front boom struck his right leg. His right ankle was sprained. He was on restricted duty from 2/12/2007 until 2/19/2007, at that time he returned to full duty.

employee was riding in 4 wheel buggy and caught his left ankle between the rib and the buggy frame.

EE was riding in mantrip when a locomotive and the mantrip inadvertantly collided on track at #4 head drive. The key steel fell out of front drive unit on a hill and caused the mantrip to slide back down the hill striking the following locomotive. EE hurt his right knee and ankle.

EMPLOYEE STRAINED HIS NECK WHEN TWO RAILRUNNERS COLLIDED ON A CURVE AT THE #32 BREAK ON #3 MAINS. NOTE: EMPLOYEE WORKED UNTIL 7/12/04, THEN STARTED MISSING WORK DUE TO INJURY.

EMPLOYEE STRUCK THE INSIDE COMPARTMENT OF THE DIESEL LOCOMOTIVE AND FRACTURED HIS RIGHT ANKLE.

EE WAS INJURED WHILE REMOVING ROCK FROM THE SECTION BELT AND THE BELT WAS INADVERTANTLY STARTED.

EE STATES HE WAS OPERATING A LOCOMOTIVE WHILE ON HIS KNEES AND HIS LEFT KNEE BEGAN TO HURT AND SWELL. HE DID NOT STRIKE THE EQUIPMENT NOR DID ANYTHING STRIKE HIM.

EE ALLEGES HE STRUCK THE SIDE OF A RAILRUNNER FRAME WITH HIS LEFT ARM.

EMPLOYEE WAS SHOVELING WHEN A PIECE OF COAL STRUCK HIM ON LOWER BACK.

LOST BALANCE, STUCK HAND OUT OF SCOOP; HAND PINCHED BETWEEN BUMPER (RUBBER) AND SCOOP AT PIVOT POINT.

EE STRUCK HIS HEAD ON THE DOOR FRAME.

EE WAS STRUCK ON THE FOREHEAD BY A CHAIN WHILE REMOVING LOW STRUCTURE FROM A PREVIOUS PANEL. THE CHAIN EITHER BROKE OR CAME UNHOOKED.

SPEED REDUCER ON CONVEYER ROLLED OVER ON LEG.

EE WAS STRUCK BY A PIECE OF ROCK THAT WAS THROWN FROM THE CONVEYOR BELT.

HANGING ROPE ON DOLLY & DOLLY HUNG WHEN IT MOVEDIT JUMPED CATCHING HIS FINGER BETWEEN ROPE & DOLLY FRAME
