Employee was bolting in #5 entry at Spad # 17+60 and as employee was walking back to pull up the bolter, ee hung the right foot under a rock lying on the bottom, tripping ee causing ee to fall twisting ankle as ee fell to the ground. Employee finished the shift and didn't go to the Doctor until that afternoon.

Employee was cleaning outside widows on unit #107 777F Cat end dump, lost their footing causing them to pivot around while holding the handrail striking the handrail with their left rib area.

EE was removing block from a scoop bucket and went to walk away from the scoop and slipped off of wet and muddy cable and fell down. EE's left foot stayed stuck in the mud, when EE fell and twisted left knee.

Taken to emergency room. Employee also stated he has an ongoing back issue with disk deterioration.

EE was working on bottom floor pump in Prep Plant when he left to go see if the pump was pumping he stepped into an area where they had removed a section of the floor drain grating to clean for pump. Causing over extension of his knee. EE finished his shift and missed his first scheduled work day on 5/25/2015

The employee was cleaning the railroad track out getting ready to load a train when she slipped and fell injuring her back.

EE(ACC Employee) was working on the surface area of the Mine helping to drill a rock dust hole. As he was pulling a drill steel back onto the trailer he slipped on another drill steel that was lying on the trailer causing him to fall over the baffle board which resulted to a strain to his back.

EE was rock dusting on the #6 belt x cut #57. EE was getting out of the deck of the scoop. When all the body weight was placed on the left leg the left knee twisted.

The employee was dismounting a Cat skidder when he slipped and fell on the last step causing an injury to his back.

Employee was traveling from the parking area to the PC800 Track hoe that he was going to operate, when he reached his destination he stepped out of the F-350 man trip van and stepped into a hole or on a rock and turned his right ankle.

Employee states he was pulling up bolter cable from slack pile. When cable reel kicked in, causing the cable to strike him in the face, causing him to fall to the ground injuring his back.

EE. States: taking spill board down on belt line lost balance fractured left ankle.

Employee stated he was descending the steps on the drill when his foot slipped off a step and he grabbed the hand rail to keep from falling. He sprained his lower back. Employee said he had three points of contact and his boots was muddy because it had been raining.

while putting in belt with another employee, we lost our footing and I fell striking head on something. causing four stitches.

Employee's truck was in the maint. area, his truck had just been in for repair. He proceeded to do his pre-op check on the truck when he lost his footing and sprained his knee.

WHILE WORKING IN #4 ENTRY ON #3 UNIT, STOOD UP AND TURNED AND TWISTED RIGHT KNEE.

EE WAS LEAVING #3 UNIT AFTER STAYING IN TO ROOF BOLT AND ANOTHER EMPLOYEE WALKED UP FROM BEHIND AND GRABBED HIM AROUND NECK (PLAYING A JOKE) AND THEY FELL AND EE TWISTED KNEE.

Operator was climbing up the steps of a Cat 773 water truck when he lost his grip causing him to fall back to the ground. He tried to break the fall by throwing his arm back and fractured his wrist when he hit the ground.

Injured employee was working inside #377 752 Page Dragline. The machine was down for maintenance. EE slipped on a spot of grease and fell dislocating his right shoulder. EE was taken to Ohio County Emergency Room where his shoulder was put back into place.

employee was walking toward the bolter machine. He stepped into a hole twisting his right knee.

Employee was fueling a dozer. He slipped on frozen ground and strained/twisted his lower right leg.

The EE was assigned to service the mantrip located outside the mine shop.As he was walking out of the shop, he tripped over the corner of a wooden pallet that was laying on the ground next to the shop. While he was walking out of the shop, he was carrying a quart of oil that he was going to put in the mantrip. He fell forward & landed on his right knee & right shoulder.

EE stated that he had been in the control room area on the 2nd floor & had reported to his supervisor that he was not feeling well. The supervisor stated that the injured mineer sat at the table for 15 minutes, got up and left the room. The injured went down the 1st flight of stairs to the landing, turned & started down the 2nd row of steps not using the handrails. He fell.
