The employee met with supervisors in a parking lot on the east side of the building. After the conversation, EE turned to walk back to the office and tripped over a wheel chock (angle iron) because EE did not fully clear it. EE fell onto EE's right knee and elbow. EE was wearing steel-toe boots, and no other PPE was involved.

EE was dismounting a haul truck from night shift. EE was on the lower platform of the truck when EE's backpack became hung up on the handrail. As EE was trying to free the backpack, EE lost EE's footing and fell to the ground. EE sustained a laceration to EE's face and soreness in EE's neck. EE received 13 stitches and was taken off work.
A haulage operator was performing their walk around of the HT630 in the 15 shovel tiedown area when they stepped on lose material in the parking ditch and slipped. This caused a strain in their lower back.
While employee was walking under EE's truck on the driver's side for an air valve check, the right foot slid out from underneath EE. This caused all weight to shift to EE's left side and resulted in a left ankle sprain.

After completing shift, employee was walking to the parking lot, after a light rain shower, and while attempting to walk around puddles slipped but didn't fall. They reported off work that evening due to pain, was seen at the clinic the next morning and placed on restrictions.
Employee had finished servicing the dozer, climbed back on to remove lockout lock, slipped and jerked their shoulder. They refused treatment, but the next day took themselves to the ER, was taken off work until 5/27, was reevaluated and released to work
