Showing all 6Employee was walking from pickup to the employee change room and slipped and fell. Employee said employee was ok and worked entire shift. Woke up next day 1/9/2025 and called foreman and said was going to the doctor, that the left knee was swollen and hurting.
Employee was stepping up onto raw scales ramp and strained or sprained left knee. Employee worked up till 10/13/2022
EE was in the parts yard looking for a replacement hose and stepped on a parts pallet which broke causing EE to stumble and fall.
Employee was sitting on the back of the mechanic truck and stepped down and started to turn around when EE's ankle gave causing EE to lose EE's balance and fall to the ground. EE tried to catch self with EE's right arm causing EE's injury.
Exiting steps of lube trailer. Stepping off bottom step right foot became stuck in mud while the other foot was still on the step. Proceeded to fall backward hitting left side of torso on large mining truck wheel chock.
