Employee alleges that EE was working on anti-freeze hose and having trouble with the quick connect. as EE got the hoses connected, EE claims EE felt a pain on the left side of EE's neck. EE finished the shift and worked the next day. The pain allegedly got worse and EE went to the er to be checked out on 5/15/2026

An employee seen smoke coming from the front of truck and went and got help plus fire ext and started putting fire out used a total of 8 fire ext to put fire out and a water truck to sprays water to cool the truck. Fire was contained to the cab and engine compartment. Msha was on site at time of event.
Employee was entering the cab of dozer putting lunch box and hard hat behind the seat. EE didn't realize their hand was in door jam and dozer door closed on EE's thumb.
Employee was walking from pickup to the employee change room and slipped and fell. Employee said employee was ok and worked entire shift. Woke up next day 1/9/2025 and called foreman and said was going to the doctor, that the left knee was swollen and hurting.
Operating a Komatsu articulator truck coming from mud cell front tire dropped in a hole in the roadway and twisted the operators back.
Highwall failure on the Williamson pit hitting the highwall miner and two coal loaders no one was in the pit when failure occurred.
Employee left miner with shuttle car and hit 2 potholes in roadway and hit head against canopy of shuttle car and hurt EE's neck. Employee work up to 8/22/2024 and doctor took EE off work for two weeks.
Employee was installing 5'foot bolt in drilled hole. As EE was pushing the bolt into the resin pack, EE gave an upward shove and felt sharp pain from their right hip to the middle of the lower back. Employee was walking and did not need any assistance.
Injured Employee was bolting with another employee when employee's steel stopped up. Employee grabbed a piece on pinner steel and hit the stopped-up steel. The drill bit came off and hit injured employee in the mouth and busted EE's upper lip and broke a couple of teeth. Employee received stitches to their lip.
Was welding a crack in a hydraulic cylinder tube; hydraulic fluid leaked and caused 1st degree burns.
The electrician was in the conveyor of the #5 shuttle car lowering the canopy when a piece of draw rock fell and struck their right foot pinning it against a conveyor flight. The injured employee worked up till 10/2/2023
Roof fall on right return 1 break outby spad #753 The roof fall did not impede travel or disrupt ventilation. The fall is 20x20 and above anchorage area was bolted with 4' foot fully grouted resin bolts in May of 2007. The fall was discovered on weekly, and area was timbered off.
Based on our initial investigation, it appears a hydraulic hose ruptured on the dozer and sprayed on the turbo. The operator noticed flames coming out of the engine compartment and exited the dozer.
A Loader had caught fire in the Williamson pit. while loading a rock truck. The operator got to safety off the machine before the loader had fully engulfed in flames. Miners were unable to extinguish the fire, so miners were removed from the area and area was blocked off. no injuries were reported.
Employee was mobilizing dozer to work site for following day via haul road. Cause of accident unknown. Employee went over embankment and was found approx. halfway down mountain. Details unknown at this time. Employee found deceased. No eye witnesses.

Track came off of rail of highwall miner while tramming causing a small piece of metal to shear off striking the employee in the left leg below the knee. The employee never missed work till 5/2/2022.
Employee was working on installing a roller frame on the D-11 dozer #040, when positioning the frame they placed a hand in a pinch point and smashed fingers between the roller frame and the bogie.
Employee was using a shop hammer to remove a pin from the cutter chain on the HWM when a piece of metal coming from the pin punctured EE's right leg just above the knee.
Employee was operating a rock truck being loaded when a rock unexpectedly flopped out of the loader bucket striking the offside bed railing of the truck bouncing EE around in the cab.
Employee was cleaning up the out slope below the ALMA seam on the left hand side of Mill Seat. When the excavator took off down the slope overturning on its side down the hill. When the excavator came to rest EE took the seatbelt off and fell out of the seat striking EE's head and shoulder.
Was doing maintenance on water pump. Was moving suction line and it had an exposed wire on the bottom side. Punctured hand.
EE was in the parts yard looking for a replacement hose and stepped on a parts pallet which broke causing EE to stumble and fall.
Employee was sitting on the back of the mechanic truck and stepped down and started to turn around when EE's ankle gave causing EE to lose EE's balance and fall to the ground. EE tried to catch self with EE's right arm causing EE's injury.
