An employee fell while walking. While attempting to catch themselves, they sustained lacerations to their left hand and chin.

EE was exiting a Broderson mobile crane and caught their foot on the bottom edge of the Broderson doorway. The EE tripped and fell approximately 18-20 inches striking their left knee on the ground. The fall caused a minor scrape to their left knee, at a follow up appointment on 1/6/2026 the diagnosis changed to a closed nondisplaced transverse fracture of left patella.

Employee arrived to work for the start of day shift. On their way into the truck shop from their personal vehicle, the employee slipped and fell on ice in the parking lot and broke their hip. Employee was transported to the hospital for medical treatment.

Employee was walking outside of the unload pocket when they stepped into a wash-out injuring their lower back.

While rotating a haul truck final drive during re-assembly per service manual instructions, two employees were using long pry bars to rotate hub assembly. One of the employee's bar slipped out of its position on the hub causing the employee to fall backwards to the ground, rolling EE's right ankle causing a fracture to the employee's right tibia and fibula.

An employee was ascending a stairwell prior to the start of their shift when the employee experienced a personal medical issue. The employee subsequently tripped and experienced a stair fall impact sustaining injuries to their left upper extremities.

Employee was stepping down from the last step of the haul truck EE had in the truck shop for repair. They took the last step to the clean dry and level cement floor and felt pain in EE's right knee. Employee stated the knee had been surgically repaired from a prior incident that happened outside of work. Employee was taken in for evaluation.

Operator was cleaning tripper for maintenance. Operator was traveling on tripper walkway when the operator slipped and fell on water/ice formation. Operator sustained dislocation of their right fifth finger.

Employee was walking around a service truck and slipped on ice, injuring EE's right shoulder. EE began missing on 5/16/2025, because of rotator cuff surgery.

Loader operator tripped and fell while entering the cab of their loader, straining their neck and back.

Employee was dismounting a rock truck when EE lost EE's grip and fell to the ground.

Hernia possibly a result of previous fall at work.

While an employee was working to unload a rod car, the employee had a mis-step while transferring from one side of the car to the other and fell against the rods. The employee felt discomfort in their left side (rib area) and in their left shoulder. The employee later sought medical attention and was put on light restrictions.

Employee was deboarding water truck 5168 from the operator's cab when their foot slipped off the top step causing pain to their right hip and knee. Employee continued to work full duty until imaging conducted on 11/19/2024 revealed injury to their right knee. Employee was prescribed restrictions.

Injured was walking on uneven floor grating when they tripped and fell to their knees.

A maintenance mechanic was deboarding a CAT 24M motor grader (GR22) when they fell from the operator's cab platform to the tandem platform landing on their right knee. The employee was evaluated by a site EMT and transported to the local area hospital for further evaluation. The employee sustained right hip sprain and right knee contusion.

EE fell and landed on their right wrist while trying to avoid a light fleet vehicle that was backing up to retrieve tow slings. EE received a splint during the ER visit.

Employee states that while taking a slurry sample out of the thickener, they stepped back and their left leg went through some deteriorated grating on the walkway above the thickener. The employee scraped their left shin and twisted their right ankle. The employee was taken to a medical facility for evaluation and was diagnosed with mild sprain of ankle.

EE was climbing onto truck to open tank lid. EE's foot slipped & EE fell backward onto ground hurting left shoulder.

While checking the oil on the 529 Haul Truck, the employee missed EE's hand placement and footing while on the engine bay stairs, causing EE to fall against the truck frame/tire. EE's lower back and left leg felt sore afterwards. Employee had knee surgery 10/15/2024.

Employee slipped and fell while walking on a slope resulting in pain in the right shoulder.

Employee tripped while taking measurements which resulted in falling and cutting the employee's left knee requiring stitches.

Employee finished clearing stuck material in the pellet reclaim hopper. When descending the fixed caged ladder, employee fell approximately 12 feet to ground level. Investigation revealed a metal electrical conduit line positioned near a rung on the ladder which may have interfered with proper footing. Employee was diagnosed with a left arm/wrist fracture.

Employee stepped into a low place and felt pain in right knee. Employee has started missing work from this event.

Employee was walking along the #1 Belt on the offside, when the employee slipped on the wet uneven bottom striking right shoulder on a 6" waterline. Employee began missing on 7/28/2023 because of this injury.

EE was working near conveyor belt 10 as the vacuum truck operator. The employee was walking back to the vac truck and tripped on a rock causing EE to fall and land on EE's shoulder. EE dislocate EE's right shoulder

An employee was transferring from a fixed ladder to 28B conveyor platform when the employee slipped, contacting their right shin on the platform grating causing a laceration to the shin. The employee was evaluated at the ER and received 3 stitches for the laceration.

Pit electrician was picking up a 13.8kV cable head when force released from the cable and pushed EE to the ground, injuring EE's shoulder. EE is currently in PT.

An employee slipped and fell on ice that accumulated in the lower tunnel entrance to the plant building while arriving to work. The employee experienced left knee pain and was sent to the hospital for further evaluation.

EE was walking outside and slipped on a snow packed road, fell and landed on their shoulder causing a dislocation of the left shoulder.

Employee was walking in the contractor parking lot from their car to the company van. As employee was walking, employee slipped and fell onto knee.

Employee slipped on a patch of ice while approaching a piece of equipment to talk with the operator. Employee landed on elbow which jarred the shoulder. Employee felt soreness in shoulder. Through an MRI a torn rotator cuff was diagnosed. Surgery to repair the shoulder took place on January 17th.

When employee stepped off of the leaf spring and drag link to the ground, the left knee popped and buckled as it began to take weight resulting in pain and swelling in the affected area. Employee works 4 on and 4 off, EE began missing on January 28th 2023.

Employee was on ground using remote control to move Drill to next hole. Employee stated they stepped on uneven ground, causing their left ankle to twist. Snowing at time of incident.

Operator misstepped off of last rung while descending from 8 yard loader. Did not fall due to having 3 points of contact but abrupt motion caused acute lower back pain.

Worker fell from scaffolding ladder and sustained an injury to EE's left leg

Employee states that they were attempting to travel down the ramp between 61/161 conveyor shuttles when employee slipped on pellets and fell backwards. As they tried to catch themselves with left hand, they fractured a wrist.

Employee's foot slipped on last rung while descending ladder. Their foot landed awkwardly; they twisted their ankle resulting in a sprain.

While employee was delivering parts to the mine warehouse, the employee slipped on the snow when returning to EE's vehicle and fell on EE's right side.

While walking on a grate to return a torch EE was using, EE's feet slipped out from under and EE fell and landed on their buttocks.

Descending stairs to retrieve tools. Both hands were on the handrails and EE slipped, caught self but EE's left hand got caught on the handrail upright. EE's left ring finger was fractured right at the top of the hand.

Employee was walking from the parking lot to the main gate and slipped on some ice landing on right arm/side causing elbow, shoulder, and muscle soreness.

Employee felt dizzy while getting out of 916 water truck. Grabbed steering wheel as lost balance and felt a pain in the right arm/shoulder.

Employee was exiting the cab of a Skid-steer loader, facing the loader, when left foot caught on a cutting edge bolt of the loader bucket. This caused the employee to trip, twisting left ankle and falling to same level.

Employee was walking along scoop and got stuck in the mud, lost balance and fell into the batteries of the scoop injuring ribs.

Employee states that while stepping off a shovel bucket, they rolled their left ankle. Employee felt immediate pain in left ankle.

Employee was walking downslope at the No. 1 Drive heading to the return pump when EE slipped and strained (Left) knee. On 7/21/2021 employee began missing work because of the injury.

EE was walking to the power center from the feeder area, when EE caught EE's foot on a rock and fell twisting EE's lower back. EE has started missing work from this injury.

Employee was hosing on the feed end of the operating floor and noticed a kink in the hose. Employee pulled the hose and as the kink let loose, employee lost balance and fell to the ground.
