Tech was climbing up back of 637 Scraper between transmission and rear fender to change a pump. Tech had grease on bottom of foot and he slipped and fell backwards. As he turned to catch himself, he fell to the ground approx 4 feet. He fractured his left wrist trying to catch himself.

The employee was standing on a rock about 12" off of the ground loading secondary rounds in a larger rock behind it when his boot slipped off of the rock twisting his left knee.

Employee was stepping out of forklift, when he twisted his left ankle. This was not determined recordable until medical review on 3/19/2007, employee will be required to have surgery.

Employee was climbing down from the bed of the CC-152 boom truck when he started descending the boom truck his right foot slipped off the rung, causing his right leg to hang up between the rungs on the dismounting ladder.

Employee was climbing down the ladder of his haul truck when his foot slipped. He did not feel any pain at the time and went home to sleep. When he awoke his lower back was hurting.

Employee was walking down the stairs at the mill maintenance building when she slipped and fell landing on her back. This resulted in a strain to her neck and back.

Employee reports while descending haul truck ladder, felt pain in back of knee and fell.

Employee was on the lower rungs of a ladder in the mill grinding area when he lost his balance; his face impacted the ladder causing him to fall to the ground. This resulted in a minor cut to the chin and a strain to his right shoulder.

Mechanic was moving a bottle of oxygen and lost his footing. He fell backward onto a piece of angle iron directly onto his buttocks.

EE was stepping off the dozer & step on dozer arm was caked with mud and his foot slipped off the step. He was taken to the ER, referred to a specialist and taken off work approximately 3 weeks. Initial diagnosis: possible fracture, 2/1/07 strained ligament, MRI to be conducted. CURRENTLY ONGOING.

Employee was at the SX Central working overtime on the SU-33 water truck. He exited the driver's cab and stepped down and his left ankle twisted/popped on him.

At the truck shop in front of pad 14 while dismounting the operator of the T501 missed the bottom step and fell backwards making contact,on the cement pad cutting his head.

EE was walking out of the guard shack to assist a vehicle when he slipped on some ice and landed on his left shoulder. This weather is unusual for this area. There was also snow on the ground. EE went to the Mt. Graham Hospital to receive treatment, and was released with a bruised shoulder.

On 1/21 employee walked across the parking lot at the mine lineout room when he slipped on the icy asphalt and fell to the ground. He initially said he was OK and refused medical treatment. He worked four 12-hour shifts, and had seven days off before calling off on 2/1 because his back hurt. We sent him for medical evaluation on 2/2 at which time he was put in a No Work status.

The Operator of the 81 crawler was pulling himself up onto the equipment when the employee slipped on the wet surface sustaining injuries.

The cable crew laborers were hanging bridges on the ramp leaving the 13 shovel. As the Laborer was attempting to hold the rope while it was to be tied off, the rope started to slip. The laborer let go and started to run when injury to knee ocurred.

While working on a drill, employee tilted a ladder back and leaned it to the side. As the ladder moved, employee stepped sideways to catch it and stepped into the drill hole on the deck hitting his side of his chest.

Was cleaning and cutting concrete anchor bolts with a cutting torch. When he stood up and stepped backwards, he tripped on a concrete peir and fell. When he fell over backwards he caught himself with his right hand causing a hairline fracture.

EE alleges that on 1/12/07, that he twisted his knee while getting off of the 300 excavator, but did not mention it to supervisor until 1/29/07.

Employee was carrying a 5 gallon bucket of concentrate. He was stepping over a drisco pipe, slipped and sprained his ankle.

The employee was standing on a platform to unplug a regent valve. He went to step off the platform and his pant leg was caught on the top step. With nothing to grab onto, the employee fell to the grating. The employee sustained a laceration to his right elbow resulting in stitches.

Employee was walking from the employee parking lot to the truck shop on the access road when he slipped on pea gravel, causing his feet to come out from under him. He landed on his back and head, causing severe pain to his lower back. He was taken to the ER and taken off work through 01/16/07.

Employee was standing on the tires of the motor grader checking the oil levels when he lost his balance and fell to the ground. This resulted in a fracture to his right hip.

Employee was stepping down off a ladder. He slipped on a muddy step and sprained his right ankle.

While walking back to passenger van, employee twisted left ankle. This incident did not become recordable until 01/30/07 due review from medical.

Employee was assigned to the 570 HT was performing a pre-op inspection at the start of graveyard shift. After checking the fire supression system on the deck of the haul truck, the employee turned and fell.

Employee lost footing as he was descending a stairway. Employee rolled left foot and fell. He was taken to clinic for observation.

Employee was stepping back off the short head and lost his footing. He fell to the ground from a height of 1 and 1/2 feet.

Employee was coming down the ladder after fueling a service truck. His foot slipped off the last step and he fell on his back hitting a metal beam on his lower left side fracturing a rib.

Employee was on the CMT 4955 fixing a broken fitting. Turned to walk away and stepped on a rock and twisted to the right and fell down, twisting right knee to the outside.

On 12/6/06 employee picked up some material from the top of a pallet. When he stepped off he aggravated his right knee. He continued to work his regular duties with minor restrictions. On 3/5/07 we were informed he had arthroscopic surgery on an out-patient basis in early Feb, however he was able to continue to work his regular duties.

Employee was walking on the leach dump and tripped on a valve causing him to fall. He landed on both hands and left shoulder popped.

Employee was loading a hang up in P-87-310 draw point. A large rock was up against the brow and open on both sides of the rock. He heard some material fall and turned around to run out of the draw point when he tripped and twisted his right ankle.

Employee tripped backward over sample boxes. She did not fall but strained her back trying to keep from falling. She did not request medical care until 12/7/06.

Employee was walking down a flight of 3 steps. When employees left foot touched the bottom of the steps, his ankle popped and twisted.

While descending from ladder of haul truck, employee lost footing. He caught himself with his left arm, causing pain to arm. Incident was not considered reportable until medical findings showed injury.

EE was mounting his truck when he lost his footing, causing him to fall back, twisting his knee. He was taken to the ER and given a prescription for pain and taken off work. On going.

Employee was climbing down the ladder on the dozer when he twisted his left knee.

Employee was walking down an incline on the MFL and slipped at the bottom. His momentum caused him to flip over a 24 pipe and landed injuring his left leg and right arm.

Employee was at the raw water booster station when he stepped out of his truck and experienced a sharp pain in his left ankle.

The cleaning crew had opened 5 of the manhole covers in the mine change room to wash the floors. An Employee walked into the north end of the change room and fell into one of the open holes, The employee caught himself with his arms and prevented himself from falling through, causing the shoulder injury.

Employee stepped backwards away from a suspended load and tripped and fell on left elbow.

Employee was walking in the driveway behind #15 mill when he slipped and fell in the lime that flows into the mill building resulting in a fracture to his right wrist.

EE was trying to open the drain in the cell. As EE was opening the drain, he slipped and fell into the trench, rolling his ankle. He was taken into hospital for futher examination. EE was take n off work and given a prescription for pain. On going.

Employee was walking into RW Guardhouse and tripped over a mat. Employee bumped her knee against the door.

Employee was stepping over frame to get down from Haul Truck.

As employee was going down the stairs that crosses the down ender on the Southside tankhouse stripping machine. Employee missed a step and fell to the floor.
