The miner slipped and fell on EE's back and rolled 3 to 4 feet. The miner had right shoulder pain. Original diagnosis of right shoulder sprain on 4/24/17. The miner had no restrictions/full duty and has had no lost time from the accident. On 7/7/17, diagnosis changed to a torn rotator cuff and is scheduled for Right Shoulder Surgery on 7/12/17.

Showing all 22Miner was walking down a ramp and tripped on something causing them to fall down stairs of a cone screen. Miner said they were not sure what they tripped on and there was no debris on the walkway. Miner fractured right ankle after the fall. Miner fell approximately 4-6 steps from the landing. The miner was not carrying anything during the incident.
TRUCK HAD MECHANICAL FAILURE, MINER WAS STRUCK BY THE OPEN DOOR KNOCKING HIM TO THE GROUND CAUSING LEFT KNEE, RIBS AND ANKLE PAIN. MINER WAS GIVEN Rx FOR PAIN AND SWELLING, ICE PACK AND KNEE IMMOBILIZER.
WHEN BRINGING A SCREEN CHUTE INTO THE SHOP FOR REPAIRS THE CHUTE SHIFTED ON THE FORKLIFT AND STRUCK EE UPON THE WELDING HELMET AND SHOULDER KNOCKING HIM TO THE GROUND AND WAS UNRESPONSIVE FOR APROX 10 SECONDS. HE SAID HE WAS OK BUT 911 WAS CALLED ANYWAY AND WAS TRANSPORTED TO BE CHECKED OUT. HE SUFFERED Cervical, hip AND RT SHOULDER STRAIN
miner was standing on excavator track and while closing the door, fell off and struck his head on the ground causing lacerations to his head and hand. head laceration required eight sutures.
After trespassing onto mine property and wandering around the mine site after dark, one of the two trespassers slipped on some loose gravel and fell off of a highwall suffering severe head and body trauma.
While descending a three step ladder, subject slipped on the second rung and missed the first rung and upon contacting the ground suffered a knee strain.
When dismounting a conveyor, subject tripped and caught the handrail with his hand, causing an injury to his elbow. He did not think that it was very serious but by the time he got home that night his elbow was swollen and sore. He went to an urgent care the next day (Saturday) and received treatment for an elbow strain.

While standing on the bank of fresh water pond, employee lost balance and slid down bank, twisting ankle.
Employee missed a step exiting truck landed on shoulder.
Employee was exiting the rock picking area. When he stepped down he twisted his right ankle causing it to be sprained
The employee placed floor grating but did not secure. Came back to work Monday am and forgot the flooring was not secured climbed on it causing flooring to give way. Employee cut lip and broke tooth.
EE WAS CHANGING BELTS ON THE JAW CRUSHER, HE DECIDED TO CLIMB ONTO THE FRAME TO GET A DIFFERENT ANGLE TO WORK AND FELL OFF THE FRAME ABOUT 2 OR 3 FEET TO THE GROUND. HE HIT HIS CHEST/SHOULDER ON THE GROUND, RECEIVING A CONTUSION. EE RECEIVED RESTRCTED DUTY STATUS WHILE HEALING.
EE WAS REPAIRING A HYDRAULIC LINE ON A LOADER. AS HE CLIMBED DOWN OFF THE LOADER HE LOST HIS FOOTING AND FELL 2 TO 3 FEET. HE PUT HIS HAND OUT TO CATCH HIS FALL AND HIS RIGHT WRIST BROKE UPON IMPACT.
EE SLIPPED ON ICE AND TWISTED HIS R ANKLE. (SPLINT & RESTRICTIONS).
EE WAS ON THE GROUND INSPECTING A CONVEYOR BELT WHEN HE SLIPPED ON THE ICE. WHEN HE HIT THE GROUND HE DISLOCATED HIS RIGHT MIDDLE FINGER. THE FINGER WAS SET AND SPLINTED - RESTRICTED DUTY.
EE WAS INSPECTING THE CONVEYOR AND TRIPPED AS HE WALKED-HE CAUGHT HIMSELF WITH HIS LEFT HAND AS HE HIT THE GROUND AND SPRAINED HIS WRIST.
A BRIDGE FORMED IN THE FEED HOPPER, THE MINER SHUT DOWN THE CONVEYER AND CLIMBED ONTO THE CONVEYER TO CLEAR THE BRIDGED MATERIAL. AS HE WORKED HE STEPPED ONTO A ROLLER WHICH ROLLED OUT FROM UN DER HIS FOOT AND HE SLIPPED DOWN INTO THE CONVEYER FRAME, TWISTING HIS ANKLE IN THE PROCESS.
EE WAS REMOVING THE PAN LINERS ON THE CONVEYOR, HE HAD ALL THE LINERS LOOSE BUT TWO WHEN HE STEPPED ON A LOOSE PAN WHICH FELL OUT, HIS OTHER LEG WHICH WAS ON THE LAST PAN THEN BROKE THROUGH TH AT PAN AND EE FELL ABOUT 4' TO THE GROUND.
EE SLIPPED WHILE CHANGING SCREENS AND FELL OFF THE SCREEN.
