Employee was working on rolling up a belt on ground level. While attempting to track the belt, the hock rod used for assistance in tracking slipped, causing the employee to fall backwards to the same level (ground), injuring left ankle & knee. No initial medical attention given until 11/20/2008, where restrictions were given, identifying a MSHA reportable claim.

Performing maintenance of bushing repair to the bucket of a backhoe, the employee had stepped backward to walk away from the work area. While stepping back and turning, the EE suffered a right knee strain to his foot; his foot had slid between the two teeth of the backhoe. The backhoe was locked/tagged out with the bucket secured at ground level.

Employee identified that stepping from engine level of haul truck, on identified walk area to truck cab, he slipped on step and fell, striking his right knee on the truck. He admitted to not maintaining three point of contact, as he had his coffee cup in hand. Initial medical attention on 5/19 (not reportable from treatment of first aid)

While climbing down from the Komatsu Endloader (company number 9135), the employee fell hitting his ribs on a milk crate used for a step / platform. Fracture to the 6th rib.

WALKING TO START LOADER BEFORE ACTUAL WORKSHIFT BEGAN, HE SLIPPED ON ICE AND TWISTED KNEE. LIGHT IN AREA WAS NOT WORKING (MOTION SENSOR OUT). EE REPORTED INJURY ON 12/8/03. MEDICAL OPINION DET ERMINED THIS A REPORTABLE CLAIM EFFECTIVE 1/5/04. EE CONTINUES TO WORK FULL TIME IN REGULAR JOB (LOADING TRUCKS) UNTIL SURGERY. UPDATED REPORT AS NECESSARY.

EE WAS STEPPING DOWN FROM THE CAB OF A WATER TRUCK. HE USED THE HANDRAIL BUT LOST HIS FOOTING, AS HE FELL HIS HAND BECAME WEDGED IN THE HANDRAIL. AS HE CONTINUED TO FALL HIS HAND CAME FREE AND STRUCK THE FUEL TANK.
