Employee was working on rolling up a belt on ground level. While attempting to track the belt, the hock rod used for assistance in tracking slipped, causing the employee to fall backwards to the same level (ground), injuring left ankle & knee. No initial medical attention given until 11/20/2008, where restrictions were given, identifying a MSHA reportable claim.

Performing maintenance of bushing repair to the bucket of a backhoe, the employee had stepped backward to walk away from the work area. While stepping back and turning, the EE suffered a right knee strain to his foot; his foot had slid between the two teeth of the backhoe. The backhoe was locked/tagged out with the bucket secured at ground level.

EMPLOYEE WAS OPERATING VOLVO HAUL TRUCK BEING LOADED BY HITACHI BACKHOE (450L). THE SPOIL WAS CHUNKED AND FROZEN. THE MATERIAL JARRED/JOSTLED THE TRUCK WHEN LOADED, CAUSING A LOW BACK/BUTTOCKS PAIN TO THE EMPLOYEE.
