Struck against stationary object

Employee was walking on catwalk when they bumped into a piece of angle iron injuring a shoulder. Employee delayed in reporting the injury. Medical attention was sought 10 days post incident. Incident was under investigation until 5/19/2020

Struck by flying object

THE EMPLOYEE WAS REMOVING A WORN TRACK ROLLER ON A CAT D6 DOZER. AFTER DONNING PPE AND REMOVING THE BUSHING BOLTS, EE PLACED THE COLD CHISEL INTO THE LOOSENING SLOT AND STRUCK THE CHISEL WITH THE HAMMER. THE CHISEL SHOT BACK INTO EE'S FACE SHIELD AND LACERATED EE'S LOWER LEFT LIP.

Employee was driving a haul truck on the travelway when the truck hit a pothole causing the truck to jerk. Employee was holding handhole in cab, felt numbness of finger. Injury reported 8/15 but occurred on 8/8. Medical treatment of dianostic testing performed. Deemed recordable by MD on 9/19 with referral to PT as MRI identified bulged cervical disc. * McHenry West Pit # 26.*

Fall onto or against objects

Employee identified that stepping from engine level of haul truck, on identified walk area to truck cab, he slipped on step and fell, striking his right knee on the truck. He admitted to not maintaining three point of contact, as he had his coffee cup in hand. Initial medical attention on 5/19 (not reportable from treatment of first aid)

Struck by flying object

Attempting to remove the track pin from the linkbelt backhoe (company # 9127) the employee hit the pin with a sledge. A piece of metal broke loose and struck his left thigh, causing a puncture. Metal was removed and prescription given.

Over-exertion (Not Elsewhere Classified)

EE indicated that while trying to climb conveyor to clean out feed hopper, he knelt down onto left knee and felt pain. EE sought medical attention later and when a MRI found a tear, he reported on 5/16/06. Claim investigated and deemed actually related to work environment on 7/26/06. Surgery is needed at a future date.
