HME operator was mounting a 988G FEL at the start of shift. EE bumped left knee on the bottom 2 step ladder mounting bracket (normal bracket). EE reported knee tightened up 5 hours into the shift. First aid/home care measures were followed until employee stated they could no longer walk on that leg 2 weeks later. The determination for work-relatedness was made on 11/29/21.
Employee was exiting service truck at the shop. Employee's right foot slipped from the bottom step of the vehicle. Employee fell on right side injuring hip, ribs and elbow. No witnesses to the event. Employee diagnosed with fractured 8th right rib and contusion to the right hip.
Employee was descending the fixed ladder of a Cat 980C end loader. His foot slipped as he was descending and the employee identified he felt a strain to the right knee.
Cutting scrap steel in yard, employee stepped on a piece of protruding steel from a cable and suffered minor puncture wound to foot. Tetnaus shot given and prescription to avoid infection.
Walking under stacker conveyor, stood up (raised head) too fast and hit head on the edge of the motor frame. Injury resulted in 6 stitches.
Attempting to change out 2'x1' drettant screen on was plant, employee needed to dislodge screen with swinging 16 lbx sledge, employee missed objective/target and hand hit corner of z cross bratice. as a result, he suffered a laceration and broken pinky finger.
CUTTING BOLTS OFF UNDERCARRIAGE OF TRUCK, HOT SLAG (SPARK) ENTERED RIGHT EAR, CAUSING BURN
ATTEMPTING TO CHANGE DRAGLINE CABLE, EXERTED TOO MUCH FORCE & STRAINED HIS BACK.
WALKING TO START LOADER BEFORE ACTUAL WORKSHIFT BEGAN, HE SLIPPED ON ICE AND TWISTED KNEE. LIGHT IN AREA WAS NOT WORKING (MOTION SENSOR OUT). EE REPORTED INJURY ON 12/8/03. MEDICAL OPINION DET ERMINED THIS A REPORTABLE CLAIM EFFECTIVE 1/5/04. EE CONTINUES TO WORK FULL TIME IN REGULAR JOB (LOADING TRUCKS) UNTIL SURGERY. UPDATED REPORT AS NECESSARY.
EE WAS CLEANING UP HIS WORK SITE AS HE WAS LOADING SECTIONS OF STEEL ON THE WORK TRUCK, HE FELT HIS SHOULDERS STIFFEN & HAD PAIN
EE WAS STEPPING DOWN FROM THE CAB OF A WATER TRUCK. HE USED THE HANDRAIL BUT LOST HIS FOOTING, AS HE FELL HIS HAND BECAME WEDGED IN THE HANDRAIL. AS HE CONTINUED TO FALL HIS HAND CAME FREE AND STRUCK THE FUEL TANK.
