A miner was working from a ladder while applying a patch to a screener feed box. While reaching, the ladder slid, causing the miner to fall onto the ladder. The miner sustained a laceration to the right knee and a fracture of the right patella. The miner was prescribed medication and lost time.

During the fall, they lost right-hand grip on the handrail, fell backward, and struck the corner of a toe board with the upper left side of their back. The injury resulted in fracture of the left thoracic 8-9 ribs and a bruised lung.
A miner was performing housekeeping duties by sweeping material between two conveyors into an access hole to a bin. As they backed up and turned to get more material, they stepped into the access hole with their left foot causing their right ankle to bend. The miner was seen at a clinic and given a walking boot and restrictions.
A miner twisted their knee while moving a tank grate from a welding table to a cart. The miner was seen at a local emergency room and received a knee brace and job restrictions.
A miner was inspecting a crusher from a platform. When they stepped down to the floor they rolled their left ankle. The miner initially received first aid and on 12/3/24 went to an occupational clinic for medical treatment. The miner was diagnosed with a sprained ankle and received work restrictions.
EE was coming down from the 4th floor. Upon descending the steps from the 2nd to 1st floor, EE slipped and injured right knee. Ambulance was called and EE was transported to the hospital.
Employee was heading home after work and fractured right hand pinky finger while walking up a flight of stairs at the facility. Employee lost footing while walking up the stairs and used right hand to break their fall, resulting in the injury.
As the employee was stepping down from the side of a Rotex screener EE's right foot slipped and EE's foot made contact with a raised portion of the Rotex support leg at floor level which twisted EE's right ankle outward.

A miner was carrying their tool bag when they slipped on a wet, muddy cement pad. The miner was able to catch their fall, but strained their back. The miner was seen at an occupational clinic where they received work restrictions.
A maintenance mechanic was removing an access panel from the side of a 992G Caterpillar Front End Loader (FEL) to repair a panel securing bolt. After removing the panel bolts, the mechanic then pulled outward on the panel, lost balance and fell ~ 6ft to ground level.
While stepping over a hose a miner tripped and fell to the ground. While falling to the ground they attempted to catch themself with their left hand and struck it on a handle to an electrical box. The handle caused a laceration to their left ring finger requiring 5 stitches.

An employee was traveling north in RM warehouse/Automix area. The employee stepped in the forklift wheel tracks that were made through a film of slurried clay that was on the floor. When doing so, the employee's right foot slipped out from under EE which caused EE to fall to the ground striking their right hip on the comm. radio that was attached to the right hip area.
On 6/9/21 an EE alleged while walking across the catwalk and stepping down on the first step the employee twisted right knee. The EE didn't go the doctor but, spoke with on the site nurse. The EE continued working but, on the 14th, wanted to be seen by a doctor. The onsite nurse scheduled an appointment for the employee on June 15th, restrictions were given then.
Employee climbed up the ladder and proceeded to set the nip on the crusher. The employee then began descent down the ladder, when they stepped off the ladder to the floor, right ankle turned causing the Employee to fall to the floor. Employee went to the control room where the shift lead called 911. Employee had a lot of pain in the upper leg/hip area.
Department Helper cleaning south basement with bobcat, while exiting the equipment stepping down felt a sharp pain on left foot left side of heel.

Employee was walking to get to loader, slipped on a patch of ice and twisted knee. Employee hyperextended knee and heard a pop. Went to doctor. MRI Scheduled for 2/25/2014.
Employee had clocked out at the end of his shift and was walking out leaving the plant. He stepped down off the side walk and on to the asphalt drive. He lost his balance and fell to the right knee. He went home, went to bed and when he awoke he went to his doctor. X-rays showed a fracture to the knee cap.
Employee's knee gave out walking down stairs, no swelling to the knee., no loss time and return to work the same day.

Installing 2x4 light fixture in three offices. Shut off motion light switch button and placed tag over light switch. Then made sure no voltage on both lights and took one fixtures down. Installed new light hooked up ground wire first then second wire hot to light disconnect shock hit was approximately 2 ft. on ladder and fell off ladder. Employee taken to UAMS for observation.
Maintenance Mechanic was adjusting an extension ladder that was leaning up against the #601 Rock Breaker when his right foot slipped on ground. When he felt foot catch he wanted to protect his knee (which he just recently had replaced) and he fell to his hip breaking the top ball portion of his femur. This was a fall from same level.
Emp. reported to supervisor he had fell from 18 conveyor tripper car and fell on his left elbow. His left arm was in pain. Employee was descending stairs (2 steps) had right hand on railing and left foot missed bottom step and employee fell to ground on left arm. Shoulder may have torn rotator cuff can not move arm. Left leg had a minor scrap from step.
A catwalk broke while an employee was assisting in a screen change on 21 rotex. The employee dropped 30" to the floor and struck his right shoulder against the product chutes on the east side of the rotex.
Mechanic was going to increase amperage on a welder, on his way to the welder he stumbled over a bolt left in the floor from old construction and fell face first into a concrete pillar. The incident caused a laceration to the right cheek requiring eight stitches.

Mechanic caught foot and tripped on floor protectors located in the quarry garage. As employee turned to leave shop his foot caught the edge of the belting and caused him to fall onto the rubber. Employee's right forearm was sore as a result. Did not go to doctor immediatly
