A 3 person team was removing 11"" round metal ductwork for cleaning. A 5' section came free of the quick-camp and dropped down 5' hitting an employee in the metatarsal area of the Left foot resulting a bruise to the foot. Employee was on restrictions, and was released to full duty on 11/11/2024.

Employee walking through a doorway carrying two buckets of tools. Tripped and fell on shoulder. Initially got better then pain did not go away. Went to doctor months later and found tear requiring surgery.

During Annual Hearing Tests employee had a 25 dbA MSHA STS right ear. Followed up with employee on retraining, selecting other hearing devices, reviewed work area for opportunities to reduce noise. Documented retraining.
Employee was trying to move two chutes in the shipping basement and felt a pain in their back. Employee did not report it at that time. Employee did report it several days later. Employee does have some restrictions now.

Employee was trying to line up a bolt hole opening on 42 elevator gearbox. The employee had EE's finger inside of bolt hole feeling for when 2 sides were lined up so bolt could be installed, the 2 parts moved suddenly cutting tip of employees middle finger, employee was taken to emergency room for treatment

While employee was conducting a screen change employee came in contact with sharp edge of screen resulting in a laceration that required 20 stitches
While grabbing and cutting bags at Mixer station started to feel some pain and numbness in both hands and arms around Mid April. Employee started to stretch and treat self but symptoms have not gone away. Went to doctor and was written off for two days. Is back to full duty now.

Individual was using a 4.5 inch hand held grinder with wire wheel brush to clean mortar off of a hand trowel. Wire wheel brush caught corner of hand trowel and broke a piece off. Broken piece struck individual in back of hand causing laceration to finger.
Employee stated they bumped conveyor hood which dislodged dust/material which got into employee's eye. Employee flushed eye and still felt like something in it. Went to ER as precaution flushed eye again and was given prescription drops.
Employee was walking on top of rail car and stepped on latch which strained metatarsal part of foot. Employee went to doctor and was written work restrictions.
Employee developed an allergic reaction at work while working in coloring. Skin turned red no blisters. Employee feels it was from working with red pigment. Employee went to doctor and they wrote restrictions to stay out of area until redness goes away.
Employee reported slipping on ice and falling outside in shipping's railroad yard. Strained low back has now started chiropractic treatment.
Employee was tightening a bolt and rubbed the back of his hand on sharp area cutting the back of left hand, 27 stitches.

Employee was making an adjustment to a thermocouple on line 3 kiln exit and the Thermocouple tube caught something in kiln discharge causing tube to shift and smash / cut right thumb. Employee was brought to walk-in clinic and received 7 stitches and missing thumb nail.
While fueling truck downtown fueling station employee reached for fuel nozzle and felt pain in left shoulder. Went to doctor 6 weeks later because pain did not go away completely. Found a torn rotator cuff. Surgery scheduled for 12/19/2014.
Employee was opening a shipping tank car and strained right shoulder.
Employee was walking to get to loader, slipped on a patch of ice and twisted knee. Employee hyperextended knee and heard a pop. Went to doctor. MRI Scheduled for 2/25/2014.

Employee reported trigger finger condition to Index finger and middle finger right hand. Prolonged overuse of power tools is what doctor determined cause. Employee hand surgery to fix condition.
Emp. reported to supervisor he had fell from 18 conveyor tripper car and fell on his left elbow. His left arm was in pain. Employee was descending stairs (2 steps) had right hand on railing and left foot missed bottom step and employee fell to ground on left arm. Shoulder may have torn rotator cuff can not move arm. Left leg had a minor scrap from step.

Employee was guzling rock next to a conveyor and when he pulled on hose his elbow hit metal railing. Bumped right elbow employee did not go to doc until 11/6/2012
Jackhammer task in silicate tank clean out over 8 hour shift. Following morning very stiff back muscles, strain. Employee went to doctor week Later and they prescribed an anti inflammatory.
In the cource of The employee's normal work and over time the employee has had pain on his Left middle finger he believes with discussions with the Nurse that his normal work activities have contributed to this condition or pain. His finger does not move to straighten out. Surgery on 3/2/2012
EE REPORTED HIS RIGHT KNEE WAS HURTING AND GETTING WORSE. HE THINKS IT HAPPENED A WEEK OR 2 BEFORE (HE DOES NOT KNOW WHEN FOR SURE) WHILE HE WAS CLEANING THE SILICATE TANK. HE SAYS IT MIGHT OF HAPPENED WHEN HE WAS JACK HAMMERING. HE DOES NOT KNOW FOR SURE. HE JUST WANTED TO REPORT IT. Employee went to doc and has restrictions

While releasing air on car the employees hand brushed up against bracket and there was a sharp edge. scraped backside of hand knuckle. Infection started a few days later and he was given antibiotics
While standing on trailer tire to adjust gate, hands pulled out of gloves causing employee to fall 3 ft to the ground injuring his hip. Dislocated hip.
Employee had right wrist pain from normal work activities in maint which turned out to be Carpal Tunnel. he had surgery before he retired.
Employee bumped head on chute and strained neck. Did not hurt right away but got worse over time. Employee went to doctor and got 10 hour work restrictions.
The employee was putting away tools after finishing a job. While pulling a cart (oxy/accetlene torch kit), the cart started to tip. The employee reached over to straighten the cart and caught his right hand on the top of the cart and cut his right hand near the thumb.
Strained back while assembling hub nut on 4 Roll. Hub nut weighs about 150 lbs. A crane is used to get the hub nut close to the shaft, but then it requires a person to lift/position/turn nut onto shaft. The shaft and nut were positioned about 32 inches off of ground. In this process, employee felt a pain in low left back and down into his left leg.

Employee had a 25 db STS shift change in their hearing in both ears.
Wrench slipped off the hex nut. When the wrench slipped, the box end of the wrench made contact with the base of the separator. The employee's hand slipped, from the orignal grip position, down the shaft of the wrench at an angle. This resulted in a hyper extension of the thumb on the Employee's left hand.

Employee was shoveling up and hit a metal plate that was under a pile. Strained right wrist and arm. Was fine at time but months later employeee went to doctor and got prescription meds.
He was dumping a wheel barrow in #1 elevator, after wheel barrow was empty, he stepped back & fell. He believes he stepped on the side of his foot causing the fall. He scraped his elbow, no other injuries reported. He washed his elbow off & he said it was fine, but just wanted to report it. On June 30th employee went to doctor becuase it was infected. Antibiotics administered.
Employee noticed soreness/pain in right wrist when shoveling and sweeping. Employee stated pain is still in wrist with sharp pain going into forearm and his hand falls asleep. On 9/9/2009 employee went to doctor and recieved prescription medication and a splint.
Employee had a sore back for about one week prior to injury. Not sure how it got hurt. Then on 5/12/2009, Employee was pounding down a spout with a sledge hammer for about 45 minutes. Afterwards he felt alot of low back pain from pounding. Reported to nurse and iced went to doctor afterwards. Employee recieved 10 lbs lifting restrictions.
Employee reported injury while guzzling 2 days after it happened. on 4/8 The Plant Nurse looked at his eye it was red, irritated, and had a visible abrasion to it. She instructed employee to see eye doctor immediately. Employee was given antibiotic drops to take 4x's a day and was instructed stay out of work environment until 4/9/2009.
Fire started in dust collection duct work off of line 3 in our coloring dept. Fire lasted about 40 minutes. Fire Dept called. No injuries

Injured party was in a Per Mar Security Company Vehicle and was parked, a Front End Loader that was plowing snow backed up and crushed the front end of the truck while he was in the truck. Large piles of snow were in the area making visual difficult for the front end loader.
3M front end loader operator was removing snow while plant was shut down. A Per Mar Security guard pulled up in a truck behind loader. Loader operator backed into truck. No injuries.
While performing checks on thermocouples outside on double dump platform, employee felt something in his right eye. He thought that he was able to get it out immediately. Later that night and early the next morning the eye became irritated. Employee called his supervisor and went to his eye doctor.
Our #11 baghouse bags started on fire. The fire went on for approximately 25 minutes. Once fire was put out it smoldered for another hour as fire crews tried to gain access to the inside of the baghouse. No employees were injured.
While retrieving a pallet of chemical, an employee was backing up slowly with a forklift and struck an upright building support that was in his blind spot. The forklift came to an abrupt stop. The employee reports a burning pain across his lower back.
Carloader reported, that while closing the top cover of a railcar, he had sprained his middle finger on his right hand. He stated he was lifting the car cover with his left hand and he put out his right hand to push against the loading spout. He jammed his middle finger against the spout.
Employee was working in Shipping department and felt weak and tired. Asked to go to hospital. Diagnosed and dehydrated and suffering from Heat exhaustion. Work area or work load was not any different then normal. Was given fluid through IV and released.
Employee was walking and tripped on metal guard from concrete. Employee fell and hit mouth on railing, caused a cut on lip and broken tooth.
employee was opening the covers of a DTS rail car to begin the cleaning process. He had finished opening the third cover and as he was stepping away from the cover , he slipped and fell on top of car. At the end of his shift he talked to me and stated that he was ""still a little sore"". Employee went to doctor on Friday and is off work til Wed
Assistant carloader reported to me on 3/13 he had sore back/hurt back. On first report of injury emp. listed incident date as 3/10 and reported injury on 3/12. Employee felt it was from opening and closing railcar lids overtime repetitive.
Employee was carrying two 1 gallon buckets down stairs and slipped and fell on last step which had some oil on it. Employee fell and landed on his low back.
During normal daily maintenance activities, employee felt pain with lifting left arm in upright position; fully extended. Employee uses impact guns, wrenches when using hands squeezing and gripping devices. Pain is in left elbow, lateral to medial aspect. This seems to be cummulative/repetitive injury. Employee just went to doctor and reported on 03/05/2008.
While employee was loading 100# propane cylinder tank slipped and tipped over on loader bucket. Employee smashed right ring finger between tank and loader bucket. Employee went to walk in. Had broken finger on x-ray.

Employee was plowing snow in a front end loader when the blade of loader came in contact with a rail road rail. The loader stoped suddenly and jarred employees low back.
Employee was carrying two baghouse cages one in each hand while standing in elevator. Elevator came to top floor and did not level completely. Employee tripped on floor and fell on left knee. Strained it as well
Slurry of pigments splashed in right eye. Using air supply to flush solidified mix at bottom of mixing pot. Operator was wearing PPE. Went to hospital doctor flushed eye and administered a perscription medication.
Employee was attempting to prevent a bag from falling to the floor at which time he felt a strain in his back.

Employee felt something in eye while grinding. All proper PPE was used. Employee is not sure when dust or debris got in eye but the pain started while he was grinding. Debris in Eye. Employee flushed eye removed debris and was given perscription eye medication. Employee has no follow-ups with doctor.
Climbing access ladder to conveyor. Employee did not seek medical treatment until 8/29/06, restrictions were issued that day.
Employee was sitting in the booth, when he went to stand up his left knee locked up. There was some pain when he put wieght on it. He went to the walk-in after his shift and recieved three days light duty.
Employee was walking out the back door of lunch room to get a dust mask from safety cabinet. When employee turned to go towards the cabinet his left knee gave out. Floor is level and there were no hazards present. This employee stated that it was the same knee that was injuried in a work related accident in 1999.
A Tank Loader was injured when his right arm was pulled into #18 Tripper Conveyor between the head pulley and the cleaning brush. Injuries included accidental avulsion of some of the worker's right forearm muscle tissue. The employee is hospitalized at this time.
Employee was in men's locker room at downtown plant. He was stepping over bench to get to his locker when he hit his little toe on left foot on the bench. Employee had continuing pain ontil he went to doctor to check just in case. On 3/06/06 went to doctor and found it was fractured. No lost time or restricted duty.

Employee was salting an area by the railroad tracks due to a recent ice storm. Employee stated that while he was lifting a 40lb. bag of rock salt his right foot slipped slightly on the ice then he strained his lower back. He was put on restricted duty with a lower back strain.

While opening a top hatch of a railroad car the hatch stuck half way up and the asst. carloader jammed her right shoulder and arm. She received a strain to her right arm/shoulder and was put on restrictions.
Employee was climbing stairs with damaged safety toe shoes, left foot slipped off of step causing toes to slip out of steel toe, when he recovered by putting foot back on step the steel toe cap caught his left big toe nail ripping it loose. First aid was given. On 10-21-2004 toe nail started growing in to skin, he had the toe nail removed by a podiatrist. Anesthetic was used.
On 8-17-04 employee reported left foot pain. He stated that the only thing he did to possibly hurt it was that on 8-14-04 he attempted to switch lever on railroad track using left foot, he pushed down hard because it was stuck, he didn't feel any discomfort until 8-17. On 8-17-he was diagnosed with a bruise to the bottom of his left foot and was put on restrictions for four days.
After working with iron worker machine the employee stepped back and tripped over angle iron on floor, he did not fall but in the process he felt a strain to his lower back. He did not seek medical attention until 6-7-2004. He received chiropractic treatment and as of 6-11-2004 he was put on restricted duty
THE DRIVER WAS ATTEMPTING TO PULL OUT A METAL GRATE FROM THE ROLL FEED DUMPING STATION. THE GRATE WAS APPROX. 20"" X 36"". WHILE PULLING THE GRATE OUT, IT CAUGHT ON THE EDGE AND THE DRIVER FELT PAIN IN HIS LEFT SHOULDER. HE WAS DIAGNOSED WITH A STRAIN AND IS RECEIVING OCCUPATIONAL THERAPY. HE WAS NOT GIVEN RESTRICTIONS TO PREVENT HIM FROM DOING HIS REGULAR JOB.

WHILE DISASSEMBLING ROLL CRUSHER DUST SHIELDS WITH IMPACT WRENCH THE OPPOSITE END OF THE DUST SHIELDS BROKE FREE CAUSING THE IMPACT WRENCH TO PUSH FORWARD. HIS LEFT 5TH FINGER WAS MISPLACED ON THE WRENCH AND WAS CAUGHT IN BETWEEN THE WRENCH AND THE DUST SHIELD. HE RECEIVED A FRACTURED FINGER AND RECEIVED SEVEN STITCHES. NO RESTRICTIONS.
AFTER REMOVING AND CUTTING SEVERAL BAGHOUSE DUST COLLECTION BAGS EE COMPLAINED OF HAVING SORE WRISTS. HE WAS DIAGNOSED HAS HAVING POSSIBLE CARPAL TUNNEL IN BOTH WRISTS AND WAS PUT ON RESTRICTE D DUTY.
WHILE WALKING UP THE STAIRS TO CARLOADING STATION EE TOE CAUGHT A TREAD AND HE LOST HIS BALANCE,HE FELL FORWARD CAUGHT HIMSELF AND STRAINED HIS LEFT SHOULDER. EE WAS DIAGNOSED WITH A LEFT SHOU LDER STRAIN AND WAS PUT ON LIFTING RESTRICTIONS.
WHILE SHOVELING OUT #3 ROLL CRUSHER GUTTERS, EMPLOYEE FELT PAIN IN HER LOWER BACK. SHE WAS PUT ON RESTRICTIONS THEN COULD NOT REPORT TO WORK ON 4/8/03 (LOST TIME).
DRIVER WAS DRIVING ACROSS RAILROAD TRACKS, HIT A ""CHUCK HOLE"" AND STEERING WHEEL TURNED INTO HIS LEFT THUMB JAMMING IT. HE SOUGHT TREATMENT ON 3-5-03 AND WAS FITTED WITH A WRIST SUPPORT TO IMM OBILIZE HIS HAND AND WAS PUT ON RESTRICTIONS, I.E. NO DRIVING OF TRUCK.

WHILE WALKING AROUND RAILCAR EMPLOYEE TWISTED HIS RIGHT KNEE ON SOME BLACKTOP THAT WAS UPHEAVED BY RAILROAD TRACKS. HE DID NOT SEE MEDICAL ATTENTION UNTIL 8/5/02 & WAS DIAGNOSED WITH RIGHT MIN ISCUS STRAIN OR TEAR. HE WAS GIVEN A PRESCRIPTION & PHYSICAL THERAPY.
EMPLOYEE'S BACK GOT SORE AFTER DRIVING FRONT END LOADER AND LIFTING PIECES OF STEEL USED TO RE-RAIL DERAILED RAILCAR. HE SOUGHT MEDICAL ATTENTION ON 6-12-00 AND WAS DIAGNOSED WITH A STRAINED LOWER BACK. HE WAS PUT ON RESTRICTIONS.
DRIVER WAS SHOVELING WASTE OFF OF THE TOP OF DUMPTRUCK WHEN A WOOD SLIVER FROM THE SHOVEL HANDLEENTERED HIS RIGHT THUMB. THE SLIVER HAD TO BE REMOVED BY A DOCTOR THE NEXT DAY BY MAKING AN INCISION.
WHILE OPENING THE BOTTOMS OF A RIALCAR EE FELT A SHARP PAIN IN HIW LOWER BACK. HE WAS DIOGNOSED WITH A BACK SPRAIN, WAS GIVEN A PREXRIPTION AND WAS PUT ON WORK RESTRICTIONS.
WHILE REMOVING A BEARING FROM A ROLL SHAFT BY HITTING IT WITH A HAMMER A STEEL SLIVER SHOT OFF AND INTO EE ABDOMEN. EE DID NOT SEEK MEDICAL ATTENTION UNTIL 4-29-02 WHERE HE WAS GIVEN A PRESCRI PTION FOR AN INFECTION.
ON 4-1-02 EE RECEIVED MEDICAL ATTENTION FOR A SORE LOWER BACK ON 2-2-02 SHE PULLED ON A STEEL PLATE AND FELT PAIN IN HER LOWER BACK SHE WAS GIVEN WORK RESTRICTIONS AND PHYSICAL THERAPY.

WHILE CUTTING OFF STEEL FROM #1 ROLL CRUSHER HEART (WITH A CUTTING TORCH) SLAG BLEW BACK AT THE WELDER-MECHANIC AND ENTERED HIS LEFT GLOVE CAUSING 2ND DEGREE BURNS TO THE BACK OF HIS HAND
WHILE TURNING A 2"" NUT ONTO A SPRING CAGE BOLT A METAL SLIVER FROM THE BOLT THREADS EMBEDDED INTO THE WELDERS LEFT FOURTH FINGER. THE SLIVER HAD TO BE SURGECLY REMOVED AT THE CLINIC.
WHILE CLEANING UP SPILLED SILICATE ON FLOOR WITH SQUEEGY SOME SILICATE ENTERED / SPLASHED HIS RIGHT EYE. MILD CHEMICAL BURN AND EYE DROPS WERE PRESCRIBED. EMPLOYEE WAS WEARING SAFETY GLASSES B UT NOT GOGGLES.
WHILE DOWNSHIFTING MAC DUMPTRUCK WHEN APPROACHING A STOP LIGHT A CAR CUT IN FRONT OF THE DRIVER. THE DRIVER SLAMMED ON THE BRAKES MAKING THE TRUCK BOUNCE TO A STOP. DRIVER JAMMED RIGHT SHOULDE R DURING THIS INCIDENT. A SPRAIN WAS DIAGNOSED AND DRIVER WAS PUT ON RESTRICTIONS.
SINCE 1999 EMPLOYEE HAD BEEN EXPERIENCING NUMBNESS IN BOTH HANDS DUE TO HIS JOB AS A WELDER.ON 4/16/01 HE WAS SEEN BY A DOCTOR AND WAS DIAGNOSED WITH CARPAL TUNNEL SYNDROME IN BOTH HANDS AND WAS PUT ON RESTRICTIONS.
AFTER EE HAD TAKEN A LOAD OF COMPUTERS OVER TO (BY CART) THE PLANT OFFICES AND UNLOPADED THEM SHE STATES SHE FELT PAIN IN HER LOWER BACK & UPPER LEFT LEG. SHE SOUGHT MEDICAL ATTENTION ON 2-26. DR. STATED AGGRAVATION OF PRE-EXISTING INJURY T UPPER LEG. (RESTRICTED DUTY).
EE SOUGHT CHIROPRACTIC TREATMENTS BECAUSE OF THE FOLLOWING: WHILE GETTING OUT OF MAC DUMP TRUCK EE'S LEFT FOOT SLIPPED OFF OF THE STEP AND FELL AGAINST A THREE CEMENT WALL INJURING HIS BACK. H E DID NOT SEEK MEDICAL TREATMENT UNTIL 2-9-01. BACK CONTUSION/STRAIN - NO RESTRICTIONS.

EE WAS INSTALLING A MOTOR OUTSIDE FOR #11 BAGHOUS WHEN DUST BLEW INTO HIS EYES. EYES WERE FLUSHED AT THE CLINIC THEN HE WAS SENT TO AN OPTIMOLOGIST WHERE PARTICLES WERE FLUSHED OUT & THEN HE W AS GIVEN PRESCRIPTIONS EYE DROPS.
EE WAS PULLING ON A PRY BAR TO LOOSEN A RL TIRE TO TAKE IT OFF A ROLL HEART WHEN THE PRY BAR SLIPPED OUT. THIS CAUSED HIM TO FALL BACK ONTO THE FLOOR LANDING ON HIS BACK AND ELBOWS. ON 8-25 EE SOUGHT MEDICAL ATTENTION AND WAS DIAGNOSED WITH A CONTUSION AND BONE SPUT/CHIP TO HIS RIGHT ELBOW. NO RESTRICTIONS.
WHILE TURNING A BEARING OVER TO EMPTY OIL EE RT. HAND GOT CAUGHT IN BETWEEN THE DUST SHIELD AND A STEEL PIN. HE RECEIVED A LACERATION TO HIS RIGHT 4TH FINGER THAT REQUIRED 10 STITCHES TO CLOSE .
WHILE DIPPING APLASTIC SCOOP INTO THE BOTTOM OF A 50LB BAG OF CARBON BLACK EMPLOYEE FELT EXTREME PAIN IN HIS LEFT SHOULDER HE WAS DIAGNOSED WITHA DISLOCATED LEFT SHOULDER AND A POSSIBLE ROTATO TOR CUFF TEAR. PHYSICAL THERAPHY WAS PRESCRIBED AND HE WAS FITTED WITH A SLING AND PUT ON WORK RESTRICTIONS.
