Struck by falling object

EE reports cleaning the bottom floor of the Mogensen building when two palm size rocks fell from the Mogensen screener and hit the EE in the upper back and neck area. EE complains of continuous pain, soreness, and injury.

When other employees came to area it was discovered 1) guard was not securely in place and had been removed 2) the equipment was running at full power ~400 fpm 3) broken shovel found in pit below, 4) right hand glove was discovered on grating, 5) torch was laying on grating Employee was missing EE's right thumb. Went to ER

While prepping a conveyor belt Splice employee cut left thumb with a skiving tool. 9 stitches.

Employee was carrying 33 gallon garbage bag to throw in Dumpster and when EE lifted to get in dumpster felt a pop or crumble in right shoulder. Went to doctor and received restrictions and physical therapy.

Employee was using guzzler hose to empty #17 baghouse bin and the hose clogged then released and the hose moved forward (jumped) and took EE's legs out from under EE. The employee fell on their back hurting low back and butt. Employee went to doctor 3 days later and was given restrictions to light work.

Employee was ascending a ladder and some dust on the ladder rung fell from above and went into EE's eye. EE was wearing safety glasses. Prescription eye drops were administered at the ER debris was washed out

Employee was walking back to a Dump Truck and a tree 12 inch diameter fell on employee fracturing the fibula lower leg and a thoracic vertebra. Employee is able to move all extremities. Spinal cord is not damaged.

Maintenance employee was trying to break loose a bearing from the shaft by pounding with a hammer. A piece of the metal chipped off and imbedded in arm. Employee was taken to ER for medical attention. Object removed and 3 stitches.

Fall from ladders

Employee was working on excavator tightening bolts while on a ladder. The wrench slipped and employee fell to ground and fractured EE's left ulna and hurt EE's left hip. Employee went to walk in clinic and has restrictions.

Employee was kneeling while grinding with 6"" hand grinder. Employee lost balance. Wile trying to catch self, employee made contact with grinder.

Fall from machine

Driver was exiting water truck VEH961 at approximately 4:00pm, driver hit head on the top of the door way causing ee to miss the grab handle and fell to the ground on ee's back. Went to doc and was put on 10lb lifting restrictions ee could still do normal job.

Employee was changing steel on 2 roll. In the process of pulling NW pin on (north) roll, the pin dropped and pinched right ring finger tip between pin and hole in pulley. Employee had to have several stitches, EE returned to work after seeing Doctor. Employee was wearing gloves.

Employee knelt down to show a fellow employee something on the scissors lift. Employee then stood up and felt severe pain and described the situation as ""my back went out"". Employee was released from work and went to visit a chiropractor.

Employee was removing an ice chunk off of 10b conveyor and it slid off contacting employee's shin. Fractured tibia bone.

Employee reported waking up with a sore back. EE thought it happened when changing separator u-joints. I asked employee to stretch and put ice on if needed. Plant nurse was contacted to talk to employee.

As the loader operator was backing out of muck pile ee began to articulate the loader to square up with it. In this process the loader bucket came in contact with the back corner (driver side) of box. No damage occurred employee felt pain in neck and back

Struck against stationary object

Employee was changing cutting edge on veh 916 loader. While reinstalling bolt cut left index finger on sharp edge of bolt. Employee was taken to emergency room and received 11 stitches. Employee was wearing gloves.

Mechanic went home after shift and woke up with a sore right shoulder. Employee came to work the next day and reported injury. The day before the Mechanic was using a 3 pound hammer to pound a wedge in McCawber dome valve to loosen seal. Supv called plant nurse to talk to mechanic Employee is following up with Doctor today

Over-exertion in pulling or pushing objects

Employee was pulling an MCC Starter bucket and forgot to unscrew one securing screw. When employee tried to pull bucket ee strained neck and shoulder. Employee did not report until ee saw doctor because ee had other issues with back and neck that may had been the problem

Inhalation of radiations, caustics, toxic and noxious substances

Electrician working in electric shop complained of shortness of breath and lung issues after smelling other maintenance employees torching on a roll steel. Initially he though it was a cold or virus triggered his issues but issues have not gone away. He now is thinking an exposure to dust particulates triggered his symptoms.

Employee was sitting in lunchroom waiting to punch into work. Employee had heart attack and Plant ERT began CPR and attached AED to victim. Employee was shocked twice and became conscious again. When 911 arrived employee was talking and doing fine. Ambulance took employee to hospital stints were put in. Employee was back to work on 11/10/2015. Non work related Heart Attack.

Employee was emptying out 2 QT sly bin and using 3 lb sledge hammer. As he was pounding on bin he felt a pain in his right shoulder and back. Went to doctor on 05/28/2015 and has slight tear in shoulder. He believes it is from this incident. Now he is on light duty and therapy.

Employee was pulling wires (working off of a ladder) and while pulling the wires the ladder shifted. A few minutes later the employee felt a strange sensation in the navel area. Check with doc and said to do nothing at time over past years he has had pain and now had hernia surgery.

Maintenance employee working on Gyro crusher looking up in ceiling and some dust and debris fell in eye while wearing safety glasses. Employee flushed eye right away. On Saturday 8/31/2013 employee went to doctor and had small metal sliver removed because eye did not get better.

While driving 813 Haul truck employee experienced low back pain and numbness in legs during normal job duties. Operating conditions normal as any other day. Employee noticed that the seat was at its end of life and hand no more cushion over some parts of the seat. Employee reported injury on 08/15/2013 and went to doctor because pain increased. Employee is now off work.

Maintenance Mechanic was adjusting an extension ladder that was leaning up against the #601 Rock Breaker when his right foot slipped on ground. When he felt foot catch he wanted to protect his knee (which he just recently had replaced) and he fell to his hip breaking the top ball portion of his femur. This was a fall from same level.

driver picked up wheel chock felt pain in left lower back. Was ok until sat 24th and he woke up with a lot of pain went to doctor and he was wrote of work for one day. then sedentary for until next visit. Prescription meds administered as well

Mechanic caught foot and tripped on floor protectors located in the quarry garage. As employee turned to leave shop his foot caught the edge of the belting and caused him to fall onto the rubber. Employee's right forearm was sore as a result. Did not go to doctor immediatly

The 2 tasks that contributed to the injury were; 1)Scraping snow balls off of stacker hopper bars while reaching out from catwalk. 2 )While cleaning out catwalk around 10 conv. tightner employee was shoveling and was prying a frozen chunk of mud and rock and felt a significant sharp pain in right side of low back.

Using the Grader to plow snow from the quarry haul road. He felt a loss of hydraulic pressure in the machine. The operator stopped the machine immediately to investigate issue. As he stood up in the cab and turned around the employee stated his foot got hung up on the cab door threshold causing him to fall out of the machine onto the ground (landing on knees/legs).

Installing liner with winch and snatch block. Pulling liner into position. Left hand placed on winch cable. Load jumped further than expected and burr on cable caught glove pulling tip of left pinky finger in snatch block. Crush injury of tip of left pinky finger.

Employee was diagnosed with Carpal tunnel several years ago (10+) Most recently in Dec. 2008 he reported pain using a 1"" impact wrench to adjust the trunions on the dryer during a pinion chan ge. The job required him to use the impact wrench for 3-5 minutes at a time on and of for about 1/2 hour. Now he is going out for surgery.

While pulling the hood open on the peterbuilt truck, felt a pain in his right shoulder. Hood was slightly stuck, so he pulled harder to get the hood open. Sent driver to to see nurse and recommended ice. Driver stated it is only 20 degrees outside, so every thing is pretty stiff on truck.

While drilling a hole in no. 2 oil tank cover with a milwaukee 1/2 inch drill. The bit caught and the drill spun causing strain on left thumb.

Fall from machine

Employee punched out from work. While driving home he left parking lot and was driving on paved quarry road. Almost hit deer and crashed bike. Employee suffered scrapes bruises to head face and arms and was brought to ER by ambulance. He returned to work on July1.

Driver was getting out of truck and slipped. He grabbed on to the handle on outside of vehicle with left hand to prevent falling. Employee heard pop in left shoulder. Strained left shoulder.

The employee saw her own doctor to investigate a sore shoulder that she thought might be related to sleeping. The doctor suggested that it might be related to work. When the employee heard that information, she remembered driving the secondary waste truck over unpaved roads at Greystone were particularly rough due to frost heaving. Workers comp has said it is work related now.

1 1b conveyor belt started on fire. Not sure of exact cause at this point. Belt stopped and motor kept running. Fire Dept was called. Fire lasted appoximatly 30 minutes. No one was injured or entrapped.

Employee was cutting used mantel steel off with a plasma torch. Employee doesn't know at what point in the cut, that the eye felt like something was in it. Employee was wearing safety glasses and welding helmet at the time. At time employee did not go to doctor. Several days later he did. Was given eye drops

He climbed up on vehicle using handle and foot slipped. Employee continued to hang on handle. Felt a (pop) strain in right bicep muscle.

Mechanic was removing transfer case from 992 weld truck. When transfer case was supported by jack and ready to be lowered, the transfer case shifted slightly. He pulled his right hand back towards his body and caught a sharp edge or burr on the underside of the front part of jack. The employee lacerated the right thumb palm of hand requiring 10 stitches.

On loaded trip with finish truck, he crossed over train tracks to slow and jarred his back. This happened about 6pm on 6/14/07. He reported it on 6/20/07 when treating it himself didn't work. (ice & aspirin). At the time he didn't think it was anything to worry about, but pain did not go away.

Picking up dryer flight and felt a strain in low back. Was informed by nurse that he was being treated by a chiropracter. No lost or restricted time. Just went to chiropractor a few times and adjustments were needed.

He was lifting concave plates at gyratory when he felt pain in his lower back. He did not seek medical treatment until 9-28-05 and was put on lost time.

HE REPORTED THIS ON 3-8-04. HE IS CURRENTLY RECEIVING TREATMENT FOR A STRAINED NECK, BUT HAS NO RESTRICTIONS AND HAS NOT LOST ANY TIME.

WHILE WELDING IN A DIESETER SCREEN WITH ANOTHER EE FOR MOST OF THE MORNING THE WELDER-MECHANIC RECEIVED FLASH BURNS TO BOTH EYES. THE EE DISCOVERED THIS ABOUT 11:30 PM AT HOME. HE STATED THAT HE DIDN'T KNOW WHEN HE INCURRED THE FLASH. HE WAS GIVEN PRESCRIPTION EYE DROPS AND WAS GIVEN PAIN MEDICATION AND RESTRICTIONS OF NO WELDING FOR TOW DAYS.

WHEN REMOVING ONE OF TWO TRAMP CYLINDERS FROM A PALLET, THE PALLET TIPPED CAUSING THE OTHER CYLINDER TO ROLL OFF ONTO EMPLOYEE'S LEFT FOOT. HE RECEIVED A FRACTURED TOE AND A LACERATION HE WAS WEARING STEEL TOE SHOES.

WHILE GETTING DOWN FROM WORK AREA ON BLAST HOLE DRILL THE MECHANIC CUT HIS RIGHT HAND ON A HOSE CLAMP. THE LACERATION REQUIRED NINE STITCHES.

WHILE CLIMBING DOWN #935 MAC DUMPTRUCK, EE'S LEFT FOOT SLIPPED OFF OF THE TIRE CAUSING A SPRAIN. EE DID NOT FALL AND IS NOT SURE IF HIS FOOT WAS INJURED ON THE TIRE OR WHEN HE STEPPED ON THE P AVEMENT. HE WAS PUT ON RESTRICTIONS FOR APPROX. 5 DAYS.

WHILE UNPLUGGING CHUTE WITH A METAL ROD THE SUPERVISOR STUCK HIS HAND UP PAST THE PNUEMATICALLY OPERATED GATE. THE GATE THEN CLOSED ON HIS RIGHT WRIST CRUSHING IT. THERE WAS NO FRACTURE BUT TH ERE IS POSSIBLE NERVE DAMAGE SUPERVISOR IS ON RESTRICTED DUTY.

EE WAS CLIMBING DOWN FROM IH TRACTOR AND STEPPED ON A FLOOR CREEPER FELL TO THE FLOOR AND SPRAINED HIS LEFT ANKLE. HE WAS PUT ON 2 DAYS OF RESTRICTED DUTY.

AFTER CUTTING A HOLE IN A BRICK WALL TO INSTALL A VENT PIPE EE BRUSHED OUT THE CONCRETE DUST. THE DUST WENT INTO HIS EYES. ON 3/17/00 HIS RIGHT EYE WAS VERY IRRITATED. HE WENT TO THE DR AND WA S DIAGNOSED WITH AN ABRASION TO THE RT CORNEA. EYE DROPS WERE PRESCRIBED.

EE WAS ATTEMPTING TO TAC WELD A NUT ON TOP OF SOME CONE STEEL. HE LINED UP THE NUT & THE WIRE FEED PUT HIS WELDING SHIELD DOWN, HE STARTED TO WELD THRU THE WIRE BACKED UP HE HIT THE FEED AGA IN WITHOUT CHECKING HIS ALIGNMENT THEN THE WIEREWENT THRU HIS GLOVE & INTO HIS LEFT THIRD FINGERON 2/29 HE WAS DIAGNOSED WITH AN INFECTION & TREATED

EE WAS WALKING AROUND 1B CONVEYOR HEADSECTON TO INSPECT RETURN IDLERS. HE NOTICED THAT SOMEONE HAD NOT REPLACED THE 24"" X 24"" GRATING TO THE ACCESS TO THE PLATFORM BELOW. INSTEAD OF REPLACING THE GRATING, HE WALKED ON A 9"" LEDGE BETWEEN THE HEADSCREEN AND THE FLOOR OPENING WHILE HE DID THIS HIS RIGHT FOOT SLIPPED, THEN HE HIT HIS LEFT SHIN ON GRATING, LACERATION REQUIRED 4 STITCHES

Over-exertion in pulling or pushing objects

WHILE EMPLOYEE WAS PRYING OUT A SIDE RIAL IN THE SCREENER HE FELT A PAIN IN HIS NECK AND LEFT SHOULDER ON 3-1-00 HE SAW A DR AND WAS DIAGNOSED WITH A STRAIN. NO RESTRICTIONS. ON 3-28 HE HAD A RECHECK AND PHYSICAL THEROPHY WAS PRESCRIBED AND FURTHER TESTING WILL BE DONE.
