Employee stepped out of stockpile truck, slipped on ladder hitting left leg (shin area) on truck bed.
Employee stepped out of stockpile truck. Employee thought they stepped on rock and lost their balance. Employee fell on left arm to break fall. Then felt short of breath. Went to hospital ER to get checked.
While exiting the loader EE slipped from the ladder, landing on EE's feet on the ground.
He picked up box of repair parts and tripped right foot on hose. Fell on right knee suffering knee sprain.
On 5-4-07 slipped off step while climbing off water truck. Caught self with right arm to avoid fall, straining shoulder. No lost time as tried therapy to rehab shoulder. Did not respond so Worker's Comp authorized shoulder surgery on 7-23-07. Return to work date not determined at this time.
Soil conditions were damp/wet which contributed to employee slipping and falling. Employee struck his back, sustaining a soft tissue bruise.
EE SLIPPED & FELL WHILE EXITING HAUL TRK. WAS TREATED FOR DEHYDRATION, CUTS & BRUISES. RECEIVING 3 STITCHES.
EE WAS TIGHTENING PIPE ON WATER TRUCK. WRENCH SLIPPED AND HE FELL BACK TWISTING BOTH KNEES.
EE WAS CLIMBING UP STEPS TO 980C LOADER. HE WAS CARRYING GREASE GUN AND 3 TUBES OF GREASE. ON 2ND STEP UP HE LOST HIS BALANCE AND FELL BACK LANDING ON HIS DERRIERE. THE NEXT DAY HE HAD OCCATIO NAL BACK PAIN AND THE 2ND DAY (SAT) HE WAS UNABLE TO WALK COMFORTABLY.
EE WAS EXITING STOCKPILE TRUCK AND THOUGHT THERE WAS ANOTHER STEP. HE FELL ON HIS SIDE BRUISING HIS HIP, ELBOW, AND SHOULDER.
