A rock wedged in feeder, employee was using a sledge hammer to break the rock. The hammerhead came off handle mid swing and their right hand ring finger slammed down and hit the rock and steel causing fracture to finger.
Haul truck was ascending the ramp from the pit to the feeder. Forward motion stopped and the truck traveled backward into the berm. It laid over on the right-hand side. The operator hit shoulder during incident.

Employee was holding slug wrench while tightening crusher bolts. Another employee hit wrench. IE lost control of IE's grip on wrench and smashed left hand between wrench and crusher.

Strained right knee climbing into back of pickup truck on 3-13-17. Had swelling and discomfort, saw doctor, recommended physical therapy with no lost work time. Condition persisted resulting in decision to have surgery on 6-7-17.
Employee using 5 ft pry bar to position crusher toggle inside primary jaw crusher. Toggle shifted into place unexpectedly. Left index finger was pinched against crusher frame causing laceration and non-displaced fracture.

While running Hydraulic stone splitter, when blade of hydraulic stone splitter was engaged (depressed) the stone sprung back and smashed left pinky finger on left hand and smashed nail, resulting in removal of finger nail with 5 stitches.

On 5-4-07 slipped off step while climbing off water truck. Caught self with right arm to avoid fall, straining shoulder. No lost time as tried therapy to rehab shoulder. Did not respond so Worker's Comp authorized shoulder surgery on 7-23-07. Return to work date not determined at this time.

ON WED 09/15 EMPLOYEE WAS HELPING TO WELD. HE REPORTED TO WORK ON THURSDAY & SAID HE HAD HURT HIS EYES YESTERDAY & WORKED FOR APPROX ONE HOUR & WENT HOME. HE WAS TOLD MORE THAN ONCE BY HIS SUPERVISOR TO GO TO THE DOCTOR BUT SAID IT WOULD BE OK. HE DID NOT SHOW FOR WORK ON FRIDAY & WORKED A FULL DAY OF MONDAY BUT FINALLY WENT TO DR ON TUESDAY TO FIND THAT HE HAD A WELDING FLASH.
