While installing new wipers on the tail-end of the conveyor belt miner lost balance and bumped into the cover bracket receiving cut to the left arm.
Employee was walking from the office to the employee break room near the end of the shift and slipped or tripped and fell contacting the wall of the break room.
Employee parked the pick-up next to the drill to check on driller. The employee went to open the back door of the truck and slipped and fell to the ground on ice. Employee tried to catch self with left arm to break the fall. Employee requested follow up. Has been prescribed Physical Therapy on 4-16-19.
Employee was attempting to climb a ladder to maintain a jaw crusher. When ee stepped on the third rung of the ladder, the ladder twisted causing the employee to lose balance and fall to the ground. The employee put out left hand to try to catch self, and hurt left wrist when ee impacted the ground.
He was using the blow gun to clean out pipes - pipe was clog causing it to blow out and employee lost his balance and hit a beam injuring his neck, left upper back and right are
Employee was performing maintenance troubleshooting on a Rotex. Upon climbing down onto the platform, he placed his foot onto a chunk of dirt causing him to lose his balance causing pain in his right knee. He was treated by the company Doctor and diagnosed with a sprain.
While descending a 4 ft step ladder with one foot on the first rung, and the other foot on the second rung, the employee's boot got hung up on the step causing him to loose his balance and fall to the grated flooring below. He was transported to the company Doctor, diagnosed with a fractured 8th rib on the right side, given prescription medication and work restriction.
Employee was walking from his workstation to the bathroom when he slipped and fell on a concrete surface. The employee broke his ankle in the fall. He was transported to the hospital for treatment.
EE did not use his safety equipment as required by company policy. He had on his harness and tie off cables were attached to his harness, he did not tie off. He fell and broke arm and bruised hip.
Employee slipped and struck his knee causing a fracture.
Left foot slipped on granite gravel, fell hitting & cutting right forearm on a metal bumper
EE was walking through plant 2 on her way to the lunch room for lunch break. She slipped and fell on some loose proppant on the floor, suffering a lumbar strain and contusions to her left shoulder and upper body.
EE was cleaning loose material off the floor in the forming area. He had formed a small pile of material to shovel into the hopper and when he attempted to step over the pile he slipped and fell, fracturing his wrist. (EE terminated following positive drug test.)
EE was replacing head gaskets on 100 ton truck. After changing head gaskets, mechanic was in the process of bolting on the intake runners. To access the intake runners, the mechanic placed his left foot on the back motor mount. The mount still had the motor mount in place. The cap turned, causing the EE to fall approx 18". This caused a hairline fracture to the metatarsil bone.
EE attempting to step down from Ballast Loading Dock to track level(approx. 26" verticle drop in elevation). Lost footing causing him to fall backward. Attempted to brace fall by extending right arm/hand to contact ground on loading dock. When hand contacted ground, EE felt pain in right arm. Completed shift. Saw DR. 11-25-08. Rx prescribed, X-ray & MRI ordered. Restricted Duty
EE slipped on the ice and fell backward, striking the back of his head on the ice covering the ground. He received 7 stitches to the back of his head. There was no lost time. No restrictions. He returned to normal duty his next shift.
EE was climbing up ladder preparing to perform maintenance on sheave for No. 1 Screen conv. in the ballast plant. He decided to climb down the 12 ft straight ladder to access area from different location. As he was climbing down, the ladder slipped at top causing ladder and he to fall to ground and his right knee. He was seen by dr. and released for restricted duty.
Employee was standing a concrete equipment pad performing maintenance on the P-2 ball mill lube system. When he stepped down to the catwalk approx. 24" below, he felt pain in his foot. An X-ray was inconclusive so he was treated for a sprain & this incident was classified as first aid. On October 17, a new X-ray revelaed a fracture in the 4th metatarsal so now reportable.
Operator was waiting for truck to have oil chg completed. Went after a ladder to climb onto truck. Walked back to trk with ladder, attempted to step over a compartment cover left on floor, he tripped, fell, striking rt leg on the cover. Received cut that required 8 sutures to close.
This accident occurred 3/16/07 and became a lost time on 4/20/07. Employee was carrying oil pump to different location in mobile equipment shop. He stepped in a small crack in the concrete floor, lost balance and fell. Pump landed on palm of left hand. Employee received medical attention and was released. Additional medical diagnosis on 3/4/07 indicated surgery required on thumb.
Employee was doing pre-shift inspection on a yard tractor; lost his balance while lifting the hood back and fell backward, causing the hood to strike him on the right shoulder.
EE SLIPPED AND FELL WHILE ATTEMPTING TO ADJUST THE FORKS ON A FORKLIFT.
INJURY HAPPENED IN 2 STAGES. EMPLOYEE WAS CLEARING AN OBSTRUCTION FROM THE KILN DISCHARGE CHUTE. THE OBSTRUCTION BROKE FREE & HOT ORE CAME OUT RAPIDLY. THE EMPLOYEE TURNED QUICKLY TO RUN AND F ELT SOMETHING POP IN HIS RIGHT HIP. 5 DAYS LATER THE EE WAS CLEARING STUCK MATERIAL FROM THE CHUTEE WITH A ROD. AS HE RAISED THE ROD WITH BOTH ARMS HIS RIGHT HIP DISLOCATED & HE WENT TO HIS K
EE WAS WALKING IN PLANT 1 BAGGING/LAB AREA WHEN HE SLIPPED ON LOOSE PROPPANT IN THE FLOOR AND TWISTED HIS KNEE.
INITIAL INCIDENT WAS INVESTIGATED AS A NEAR MISS ON 10-28-02. EMPLOYEE DID NOT SEEK MEDICAL ATTENTION AND CONTINUED REGULAR DUTIES. ON DEC 2, EE WAS SEEN BY A DOCTOR DUE TO PAIN AND SWELLING I N KNEE.
FELL ON CAT WALK FROM ORE SPILLAGE ON WALK. THE ORE SPILLAGE WAS CAUSED BY THE CHOKE UP IN PROCESS CATCHING 5TH FINGER LEFT HAND INGRATING ON FLOOR.
EE HAD FOLLOWED INCORRECT MIXING PROCEDURE AND PUT CLAY INTO A COLOR BATCH AS THE FIRST INGREDIENT. THE CLAY PLUGGED UP THE BOTTOM OF MIXING TANK. EE WAS CLEANING TANK, SLIPPED AND FELL AGAINS T A TANK BAFFLE WITH HIS HAND. HE WAS NOT WEARING HAND PROTECTION. RIGHT HAND WAS LACERATED BETWEEN INDEX FINGER AND THUMB.
EE CAUGHT HIS LEFT FOOT ON 6" AIRLINE CAUSING HIM TO TRIP AND HIT HIS RIGHT KNEE ON FLOORING.
EE WAS EXITING LOCAL HARDWARE STORE AFTER PICKING UP MAINTENANCE SUPPLIES. HE FALL TO RIGHT LEG FOLDING IT UNDER HIM WITH HIS BODYWEIGHT LOCAL CLINIC TOOK X RAY WHICH WAS A FRACTURE TO RIGHT K NEE CAP. THEY REFERRED HIM TO ORTHOSCOPIC SURGEON.
