Struck by flying object

Miner working in the Line of Fire. Pressure Release Valve (pop-off) failed; thus, Pressure Transducer broke at fitting, hitting employee in the Face. Miner went to local ER for Evaluation. ER cleaned contusion, x-rayed face/head arear, gave tetanus and antibiotic shot to prevent infection.

-Miner Working in Line of Fire. -Overconfident in the task. -Did not follow company standards; wrong gloves for the job, did not do a 60's to think before doing the task. Employee was then taken to local ER where employee received 4 stitches in right index finger.

Employee working in the ore shed found a hose needing replaced. It was very rigid & bent into an odd position, so the hose was cut out. With the hose disconnected,the employee held it with left hand while holding the grinder with right. The grinder had a kick back & fell from hand & made contact with left forearm & resulted laceration.

Struck against a moving object

FEL operator at the crusher was in the process of positioning the bucket over the side of Komatsu truck when EE hit the lever to come down slamming the bucket onto the side of the truck. FEL operator was talking on the 2 way radio to tell another truck to go get material from the plant when the accident happened. Operator having the mic caused EE to push it too far forward.

Left Ankle Sprain. Causation; unknown Employee was working on a D10T and noticed EE's ankle was starting to hurt. No specific incident. Pain worsened and reported to EE's supervisor next morning. Did not receive medical treatment until 11-13-16. In which was put on restricted duty.

3 maintenance EE's were working on replacing an O-Ring on a Dozer. One EE bumped the engine to force the fuel out of the cylinder, when the fuel entered the IE's right palm but did not exit out the other side of his hand. The pressure created 2 small cuts 2 inches long on different locations on the palm of the hand. Unknown where IE's hand was positioned when the injury occurred.

Fall from machine

EE was replacing head gaskets on 100 ton truck. After changing head gaskets, mechanic was in the process of bolting on the intake runners. To access the intake runners, the mechanic placed his left foot on the back motor mount. The mount still had the motor mount in place. The cap turned, causing the EE to fall approx 18"". This caused a hairline fracture to the metatarsil bone.

Operator was waiting for truck to have oil chg completed. Went after a ladder to climb onto truck. Walked back to trk with ladder, attempted to step over a compartment cover left on floor, he tripped, fell, striking rt leg on the cover. Received cut that required 8 sutures to close.

Struck by falling object

EE was changing C-1 belt in the primary crusher pit. He attached a 1.5 ton chain hoist to a I beam to raise the new belt. The hook on the chain hoist was positioned on the I beam at an angle. When the mechanic pulled the slack out of the hoist, the head of the hoist turned allowing hoist to fall hitting EE in head & face. He sustained 2 cuts on upper lip requiring 5 sutures.

Employee was cribbing up, off road truck with crossties, to remove tires & wheels, one tie about 3-4 feet long fell over, employee bent over to pick up, tie was wet, gloves were wet & it slipped in hands, causing employee to grab for it. When he caught the tie he felt a twinge in his back.

CRUSHER WAS BEING REPAIRED, A PIECE OF METAL FELL FROM ITS RESTING PLACE AND STRUCK EMPLOYEE ON HARD HAT. THE BLOW UNBALANCED THE EE , HE FELL AND STRUCK HIS HEAD ON A STEEL FRAME, LACERATING HIS SCALP.

A TRANSMISSION WAS BEING LIFTED BY A CHAIN HOIST. EE TOUCHED THE CHAIN AND IT SLIPPED PINCHING HIS LEFT PALM.
