Caught in, under or between a moving and a stationary object

The maintenance employee was lifting a section of cutting-edge steel using a pry bar. During this process, the cutting edge shifted unexpectedly, pinching EE's finger between the edge and the bolt. This resulted in a small external laceration on the outside of EE's finger.

Employee started job at around 12 Noon, the job was to replace the flashing on belt 3. Employee locked out the belt & tried to start, after test employee grabbed the tools required to do the job, entered the manlift, put it into position & started removing the bolts on the guard. When removing employee felt pop in right shoulder. Reported it to the Mine Manager & Safety Manager

While installing new wipers on the tail-end of the conveyor belt miner lost balance and bumped into the cover bracket receiving cut to the left arm.

Left rear inside rear tire on a 777 cat haul ruptured.

Maintenance employees removing the right rear leaf spring from a lube truck. Spring had been unbolted and was being lifted out by 3 employees. The employee at the edge of the truck was told to let go. The spring rolled on its side, pinching another employees small finger causing a small cut requiring two stiches.

Removing windshield from truck. Cut left index finger with razor knife. Didn't file this report due to lack of knowledge - Accident was reported to Workers Compensation Fund of Utah.

Struck by falling object

DRILL CENTRALIZER MALFUNCTIONED ALLOWING DRILL STEEL TO DROP. THE DRILL STEEL WAS BEING LIFTED AND ALIGNED INTO THE CENTRALIZER WHEN IT SLIPPED THROUGH THE CENTRALIZER AND A RESTRAINING ROPE.

EMPLOYEE STATES: WHILE CLIMBING DOWN FROM SERVICE TRUCK #TR02-025, HE STEPPED ONTO UNEVEN GROUND CAUSING HIM TO TWIST HIS ANKLE.
