Showing all 7EE was painting on processing plant and felt his knee pop when raising from a squatting position. Cont's. to work without restriction on 4/9 & 4/10. Became reportable on 4/11/13 when EE went to doctor due to discomfort. Doctor requested that EE not work for 5 days.
EE slipped and fell while performing maintenance work on 2/14/2012. This became reportable on 4/3/2012 when he went to doctor for the first time and doctor requested physical/occupational therapy after work hours with no work restrictions.
EE was performing preshift inspection on Komatsu dozer. He had to add antifreeze. After adding fluid he stepped down off dozer arm onto ground with 5 gal. can in hand. He turned ankle. Ground was level & dozer was parked on rock road. There was light rain the night before. EE wears slip on steel toe boots. They did have a coat of lime mud on bottom.
EE WAS STEPPING OFF LOADER, SLIPPED AND FELL, HITTING HIS KNEE ON LOADER STEP.
EE WAS CHANGING SCREENS ON A SCREEN DECK. HE PULLED A SCREEN, SLIPPED AND INJURED HIS BACK.
