Injured states he was carrying a bag of B-Bond and a jug of water, he slipped in the mud and strained his groin area. The injury was diagnosed as a hernia on 10-10-14 that will require surgery to repair. The employee is currently working his regular job. The accident will become reportable as a NFDL (LTA) once he has the surgery.

IE stated that while walking to catch up with a co-worker, who was carrying a hammer, he stepped into an irregularity in the bottom causing him to stumble forward hitting into the hammer being carried by his co-worker. As he fell, the hammer struck his lip causing a laceration requiring 3 sutures.

Employee went to check the 4-inch water valve at the fresh water pond. As he turned to walk away, he tripped over the 4-inch waterline and fell onto his left wrist causing a hairline fracture that required a cast.

Employee starting to bolt an open 20 foot cut in which roof rock had been dislodged and fallen out. After setting the ATRS and while swinging out his safety post canopy, the roof began chipping and the injured was struck by a piece of falling roof rock estimated to be 3.5' long x 2' wide x 10" thick. The rock caused him to fall striking his right shoulder and ribs on the bottom.

Employee slipped off the last step while dismounting the front end loader. He initially told the foreman that he was OK and worked without incident for the rest of the week. On the morning of 3/8/09 he awoke to find his right leg swollen and black and blue. The ER @ Dubois Hospital diagnosed an infection under the unbroken skin on his leg. His first day off work was 03/09/2009.

Injured and co-workers were attempting to dislodge filter cake stuck to the inside and door openings of the stacker tube, when some loose material fell from the stacker. Injured was struck by the material, fell and rolled down the side of the 5 foot high coal pile that he was standing on. Injured had the air knocked out and felt pain in his lower back and right side.

Employee was attempting to break off a roof bolt which had bent during installation. He had used a hacksaw to partially saw through the bolt and was rocking the bolt back and forth to break it off. He gave one extremely hard push, the bolt broke off, he lost his balance falling onto the roof bolt plate causing a laceration on his right forearm.

Injured and co-workers were replacing decking material on the main belt catwalk on the previous operating shift. The workmen left this job site prior to replacing a 2' x 8' section of flooring that had been removed. The area was not gaurded to prevent entry into the area. Injured was walking down the catwalk when he fell through the hole falling approx. 9' to the ground below.
