Caught in, under or between a moving and a stationary object

Employee stated that as he was attempting to remove a rock (2' x 3' x 6-8"") from the wishbone on the ripper of the D11T CAT Dozer when the rock shifted pinching his thumb between the machine frame and the rock causing a laceration requiring 5 sutures. He is continues working his regular job.

Struck by falling object

Rocks from the side of a highwall fell striking the back of an excavator breaking the front and rear windows of the cab. One piece of rock pushed the operator seat forward requiring his seat belt to be cut off. The operator was struck by a piece of the rock in the back of his head requiring stitches.

UNDERNEATH MECHANIC TRUCK INSTALLING A VIVE ON BUMPER. HAD CUTTING FACE SHIELD ON, BUT HAD TO RAISE IT 1/2 WAY IN ORDER TO SEE. A PEICE OF DIRT OR RUST ABOVE BUMPER FELL INTO LEFT EYE.

EMPLOYEE WAS HOLDING A PIN TO AN EQUALIZER BAR WHILE ANOTHER USED HAMMER TO STRIKE THE PIN. HAMMER MISSED AND HIT EMPLOYEE'S RIGHT PINKY FINGER.

WAS REPLACING HYDRAULIC LINE ON THE SCRAPER. WHILE GETTING BACK INTO THE SCRAPER TO SHUT OFF MASTER SWITCH, EMPLOYEE SLIPPED ON PLATFORM, EMPLOYEE REACHED BACK FOR HAND RAIL TO KEEP FROM FALLI NG. NEXT DAY NECK MUSCLES WERE TIGHT AND EXPERIENCING DISCOMFORT.

WORKING ON A 4"" WATER PUMP. ICE WAS AROUND SUCTION LINE WHICH CAUSED IT TO AIR LOCK AND OVERHEAT. PUSHED FLAP IN ON PUMP AND STEAM CAME OUT AND BURNED LEFT FOREARM CLOSE TO WRIST AREA AND LEFT LEG.
