Struck against a moving object

Operating a CAT 777 Haul Truck when for some unknown reason, she stopped the loaded haul truck on an uphill grade placed the truck into neutral gear and allowed the truck to coast backward three truck lengths striking the berm causing the truck to roll over onto its passenger side. She was wearing her seatbelt and was not ejected. Accident contributed to ""Operator Error"".

Fall from machine

Employee slipped off the last step while dismounting the front end loader. He initially told the foreman that he was OK and worked without incident for the rest of the week. On the morning of 3/8/09 he awoke to find his right leg swollen and black and blue. The ER @ Dubois Hospital diagnosed an infection under the unbroken skin on his leg. His first day off work was 03/09/2009.

Employee states that he was dismounting from his Cat D10R Dozer when his feet went out from under him and he fell to the ground onto his right shoulder. He believes that he may have slipped on snow and ice on the tracks/structure.

Employee was standing next to his dozer. As he turned to talk to a co-worker, he felt a pain in his lower back. He completed his shift and went to the doctor the next day. He was advised by the doctor to take some time off work.

Employee was climbing down from a rock truck, when he missed the handrail and lost his balance, causing additional strain on his hand and arm still holding onto the handrail.

Employee was climbing down the ladder of the 777B Rock Truck to fuel up at the end of his shift, when he felt a pain in his shoulder. Employee shift started on 01/21/2008. Employee continued to work until he had surgery on the shoulder on June 20, 2008.
