Employee performing general cleanup. Began descent of stairs and slipped. EE grabbed a bracket to regain balance and received a laceration of the middle finger on the right hand. Laceration required 4 sutures.

EE was tying down & securing the train. As the EE was ready to dismount, the EE faced the ladder and placed right foot to pivot. The soles on EE's new boots gripped the catwalk not allowing the foot to pivot. Later EE felt soreness in knee. The EE was released to full duty on 10/1/18. On 11/19/18 an MRI determined a torn meniscus and EE was released to restricted duty.

EE was carrying a basket as she approached the scale house door. She began to enter the scale house by stepping onto the man-made boot wiper, pushed the basket between the scale house wall and her hip, pulled the door open and stepped forward. She lost her balance and fell forward on her right side striking the ground. The EE sustained a dislocated elbow and is on light duty.

Miner was cleaning a roller shim for roller placement. He unhooked his lanyard to move to a new location and did not stay 100% tied off. He tripped and fell backwards from the platform to the ground 14'9" below. The handrail had been removed to allow the roller installation. He broke both wrists and his left rib.

The employee was changing oil while standing on a catwalk approximately 10FT above ground, employee turned and foot slipped causing the employee to fall on the catwalk. The employee struck against the electric motor causing a strain to middle back and rib cage. The employee is seeking medical attention.

On February 6, 2014 at approximately 10:00AM, EE was working at the bucket wheel on the West transfer conveyor with 2 co-EE's. He slipped on ice and fell on his right side. He immediately felt pain in his right elbow and his lower left back. He reported this to company immediately. EE was driven to One Medical for evaluation.

Employee was walking down some stairs and stumbled. After he stumbled he felt a pain in his hip. Employee did not want to go to the doctor at this time. The pain has not gone away and the employee wished to see a doctor on 11/14/2013. Employee has been put on restricted duty as of 11/15/2013.

EE was replacing bad bolts in the SB-920. He was using an air impact to tighten new bolts, while in the process of tightening a bolt an old bolt popped loose hitting him on his right ear. When the bolt hit him on the ear he lost his footing and tried to catch himself with his left arm. He said when he caught himself he twisted his left arm and wrist.

Employee was attempting to open a valve for a back up slurry pump, but was unable to get it to open and restarted the primary pump. When pump started an over head slurry line ruptured and sprayed slurry on him blocking his escape route. The employee climbed over the hand rail and jumped about eight feet to the ground causing a fracture/ dislocated right shoulder.

Employee was walking down a flight of stairs and thought he had reached the bottom. He took a flat step but there were actually 2 more steps. This caused the employee to lose his balance and fall forward. He reached out and grabbed the top rail of the hand rail to break his fall. This caused his lower half to twist resulting in a burning sensation in his lower back.

Miner was outside of breakroom with coffee pot. Miner noticed truck in plant and hurriedly turned to go back into breakroom. When doing so, Miner tripped stepping up into breakroom and fell to the floor with coffee pot in hand. Coffee pot shattered. Miner's right hand was cut including a piece of glass enbedded in palm of right hand.

Miners were making repairs to water pump on dredge. Miner went to get crescent wrench from motorboat. When stepping from dredge to motor boat, miner slipped and fell toward welding machine. Miner' s right leg contacted the lead holder causing a laceration. Miner received 9 sutures. Contributing factors: Welding machine in motorboat.

Employee was exiting 980G loader. As he did this, his right foot slipped and went under the step, causing employee to fall to the ground while his left foot stayed on the loader step and hands slid down rail. As this happened... it hurt his left knee.

Employee received a call that work on C-7 was completed. Due to excessive noise during the call, he moved to the platform on south side of cooler to hear better and due to poor lighting, tripped over the dam sections in center of platform, falling on his right shoulder.

EE was stepping up into basket of manlift when felt a pop in left knee. EE sought medical attention on 10-15-11, diagnosis - knee sprain. Pain contiuned EE returned to Doctor and a diagnosis of Torn Meniscus and that surgery was need on 11-15-11.

EE alleges he was moving about on the ground floor of the raw mill wearing a harness. His feet got tangled up in the lanyard that was connected to the harness and fell, injuring his right knee.

Employee smelled hydraulic oil while operating 375 Cat excavator. Employee stopped running the machine and went to the back of the machine to check the fluid level. He stepped on the battery box with his right foot and while raising his left foot, he hooked it on the battery box cover latch. This caused his left foot to bend under creating a foot strain.

The ee was doing a pre-op inspection in his area when he slipped on a section of ice. The ee's feet came out from under him and he landed on his back. He was checked at the er where they determined he had strained a muscle in his back, no restrictions given. When doing a follow up 1 wk later the ee was given the restrictions of not carrying objects more than 20lbs w/his right arm.

EE's were cleaning the raw shale buildup from C-34 tripper chute. While cleaning the chute, IE was using a pneumatic chipping gun and when IE tried to change his footing he slipped and hit his left calf muscle on the inside of the chute.

Employee was unloading a piece of water pipe from a trailer. He stepped back away from the pipe when the pipe started to fall. He stepped on a rock and lost his balance and fell. He was driven to the doctor and was diagnosed with a tail bone fracture. He returned to work the same day with no restrictions.

After cleaning an air lance with a 1/8th inch rod that was in a vice, the employee went to loosen the vice and slipped on a rock. He tried to catch himself on the vice. He missed and hit his left hand on the rod causing a puncture wound and fracture.

On Jan. 4, 2010 employee was checking out the plant for start up. He was underneath the plant making sure the valves on the slurry tank and screws were closed. He went to step over the plant's 18" wide drain ditch and slipped on ice. This event caused his left leg to over extend forward. He felt pain in his upper leg.

Employee was in the quarry pit area setting off a shot and there was flying rock from the shot. EE was running to get away from flying rock when he slipped and fell due to the ground being wet from rainy conditions. There is usually never any flying rock from shots, but the wet conditions hindered the shot. EE suffered a strain in his left upper leg.

8/27/09 While stepping down from the 375 excavator, EE got his corrective boot hung in the step twisted his right knee. 8/28/09 he reported his right knee was swollen & needed go to the doctor. Taken to the doctor, told to ice the knee & he could return to regular duty. Back to the doctor 9/8/09, swelling not completely down. Dr gave prescription for anti inflammatory medication

While dismounting skid steer loader, Injured caught his right hand in between the skid bar and the roll over cab. He sustained a sprain/strain to the right hand. He was working on the finish end inside of C-21 tunnel.
