Showing 50 of 63A plant maintenance employee was splicing a conveyor belt when their foot slipped out from under them as they were dismounting off of the frame and strained their right knee.
EE was cleaning under a conveyor belt when EE stood on the bucket of the skid steer EE was operating. EE began to cut part of the belt and material fell on EE causing EE to lose EE's balance and fell aprox. 24 inches on EE's left side injuring EE's shoulder, left hip and rib.
Employee slipped coming down the ladder of a 980H. Employee landed on EE's rear end and EE's left elbow. Employee has been placed on light duty.
Employee stepped across a drainage area, slipped on some mud and fell backwards injuring their right elbow
The employee stepped off a walkway approximately one foot in height. EE stepped into some mud and material with their right foot that had collected in a drainage area. EE's foot slipped out from under EE, straining their left knee.
Employee was cleaning at the Water reclaim Plant (Sand Plant) and went to retrieve a water hose and slipped on wet slippery surface and twisted left knee.
A layer of concrete dust and debris had collected on silo roof obscuring hatches lids, and other obstructions. Walking on roof tripped on one of the obstructions. Fell to right knee and struck something in the dust and twisted some on the way down. Local clinic did x-rays but nothing broken. Received brace for sprain to unspecified site of right knee.
Contractor miner was tasked with putting the guards that go over a feed belt back on. After leaning over to tighten the guard EE relaxed their weight on the handrail and the horizontal arm on the handrail gave. The contractor then lost their balance and their torso fell on toe plate. The toe plate was 45 degrees outward when the accident occurred
Ladder not secured; working in awkward position
employee stepped up on oil containment wall. EE's boot caught the top of wall causing employee to trip and fall. Employee extended EE's right arm to catch self pulling something in EE's right bicep.
A Production Utility employee fractured left leg when the employee tripped and fell down a short set of steps. The employee had been trouble-shooting a compressor and had reset the controls. The employee stated while exiting the trailer that housed the compressor controls employee missed a step and fell. The fall resulted in a fracture to the lower left leg just above the ankle.
Maintenance Mechanic was walking towards the lubrication shack where the EE rolled an ankle due to stepping on slippery conditions, EE was then taken to the clinic for further evaluation. EE was then diagnosed with a hairline fracture in right foot.
Employee was walking through a door way when employee tripped on the landing, as employee began to fall employee reached out to stop fall by holding onto the door with right arm, when employee did this employee felt extreme pain in bicep and fell to the ground.
Employee was assigned to do an area inspection at the crusher. The employee claimed that when they exited the pickup they slipped on some mud and ice. Employee didn't notice anything wrong with the knee until 2 days later when they reported it to a supervisor. Employee requested to be seen by a physician and was put on restrictions for a strained knee.
EE was removing barricade on a slight incline when EE slipped backwards. As EE was falling, EE placed hand behind to catch self. In doing this EE injured their finger on rocks in the area.
While walking down the stairs at the K1 Homo Silo, EE stated that EE felt pain on the outside of left knee. On 09/27/2021 after self care failed, EE was seen at a clinic where EE was given restrictions and a knee brace.
The employee began to set up a 6' ladder to begin working on the belt scales. When the employee reached the 4th rung, the employee realized the ladder will not be tall enough to perform the job tasks. While climbing down the ladder, the ladder then toppled over causing the employee to fall, injuring right ankle. Employee is unsure as to how the ladder toppled over.
While walking down stairs on K2 PHT EE slipped on ice causing shoulder bruise requiring self care. On 3/8/2021 injury re-evaluated and put on restricted duty.
EE stepped off the bottom stair on a staircase awkwardly and sprained their right ankle.

Mechanic exiting cab of dump truck, felt a pain in calf that went away following day (plant wide holiday) swelling and pain that continued through the weekend. Employee went to Occupational doctor, diagnosed strained muscle. After several weeks no improvement employee went to Ortho doctor to review MRI, diagnosed torn muscle. Employee put in walking boot and work restrictions.
A production loader operator injured shoulder on March 5th when the employee tripped & fell, as employee was walking to the employee parking lot after shift. The employee was seen at the ER on March 5th & was released back to work on full duty. On March 23rd the injured employee was seen at the occupational clinic and was placed on restricted duty and prescribed physical therapy.
Employee reported slipping and falling on 01-06-2018. EE refused medical treatment and said was fine. On 3-1-2018 employee stated that the shoulder has not gotten better. We took EE to the doctor where they said EE had a contusion. EE was put on restricted duty and prescribed PT.
Employee was exiting the Loader with a piece of paper in hand when ee lost grip and fell 3 feet to the ground. Employee cut head and required 5 staples to close cut.
Employee was entering the west side of the cooler room by threw the roll up door entrance, when a chute next to the door started to dust into a containment bin, the employee quickly stepped to the side to avoid the dust, stepping on clinker material on the floor, falling and landing on left hand and knee

It had just rained and EE was walking the crusher at gray Mtn when ee slipped and pulled a muscle in left leg
EE was climbing down a ladder from the control house of the crusher and while doing so EE did not place the arch of foot on the ladder rung but only placed the front of boot where the toe's are and foot slipped and EE caught self with left hand and with the weight of body swung to the right and hit right forearm and elbow on the frame.
EE was descending a stairwell on the south end of the mill room at the end of shift. The EE thought he was on the bottom step, but actually was on the second to bottom step. The employee fell to the floor and landed on his shoulder. An MRI showed a torn rotator cuff injury and on 12/1/15 the employee was prescribed physical therapy.
IW was standing on ladder installing a part on a 777 truck bed using truck hoist. IW thought brackets were engaged (not visible while mounting part) so released tension from hoist. Using a hammer to knock part down better bracket came loose and fell knocking the ladder out from IW. IW fell to ground catching self with right arm causing injury to right arm and shoulder
Employee was descending a set of stairs when his left foot slipped off of the 4th step. The employee caught himself with the handrail and did not fall further, but he did "jam" his knee on the 5th step. The employee was taken to the clinic and was placed on restrictive duty.
EE was moving a ladder while looking up & stepping backwards. He stepped into the center of the raw mill table, falling & landing on his right side at the torso area. He came to the office at 9:09 am & said he had fallen & wanted to go to the hospital. He was having trouble taking deep breaths.
The employee stated that after climbing down the ladder of the water truck, when he went to turn to go to the back of the truck his leg planted when he twisted and he went to the ground hearing a pop in his knee. He refused to go to the doctor that day and on 12/1/2014 he asked for medical treatment and was diagnosed with a Sprain on his right knee with work restrictions.
Employee working at Cemex Balcones in Lyston Building moving rocks from Quarry to plant using skid steer. Twisted right ankle stepping down from skid steer. Taken to Texas Med Clinic, diagnosed with sprained ankle, given boot to wear with crutches as needed and placed on restricted duty. Cemex Balcones will not allow employee back to work until able to wear steel toe boots.
Employee climbed up on loader bucket to clear hopper and fell approximately 5 feet off side of the loader bucket causing a sprain to his left ankle. Employee did not follow safety procedures and was not tied off.

Employee was climbing into the feed basket of a cone crusher to inspect; he lost his footing and bumped his right knee. Employee reported the incident but felt he didn't require medical attention. Today, the employee informed me that his knee was bothering him and he wanted to be seen by a doctor, so I took him to the clinic.
employee was using a 6' metal bar to break up a build up of material in a chute. the material broke apart and the employee could not stop his momentum and his hand hit the seal for the chute door. The employee received five stitches to his right ring finger and 8 stitches to his middle finger. Both fingers were also fractured.
SPRAYING OFF CATWALK, NEEDED TO SHUT WATER OFF, PLACED HOSE DOWN WENT TO THE SHUT OFF THE VALVE QUICKLY AND TWISTED RIGHT ANKLE.

EE was walking in the plant area KT Pit #2. As he was returning from water break, EE was walking down a step when he twisted his right ankle. His supervisor transported him to a local emergency room. EE was diagnosed with no broken bones but has a twisted ankle and the dr. told him to take Advil for pain.
