EE was getting off a 980 f loader and when he got to the last step, he put his foot on the ground. he didn't see a pothole and his right ankle twisted and his weight went down on it. He went to the hospital and they said it was a bad sprain. Became a lost time on 6/17/05.
EMPLOYEE WAS WORKING OUTSIDE ON UNEVEN GROUND. HE TWISTED HIS RIGHT ANKLE. HE REPORTED THIS TO HIS SUPERVISOR BUT DIDN'T NEED MEDICAL ATTENTION. HE WENT TO THE DOCTOR ONE MONTH LATER AND FOUND THAT HIS RIGHT FOOT WAS BROKEN. THIS BECAME A LOST TIME MEDICAL ON 6/10/05.
OPERATOR WAS TRYING TO GET UP ON A DOZER, HIS RIGHT FOOT SLIPPED. HE TRIED TO CATCH HIS FALL CAUSING HIS RIGHT LEG TO HIT THE PUSH ARM, HIT SHIN BONE AND SKINNED IT WHICH LATER GOT INFECTION,CELLULITIS REQUIRING MEDICAL ATTENTION. INVESTIGATOR SAID DOZER PUSHARM NEED TO BE FIXED AND HAD BEEN REPORTED. IT WAS LEANING AND OP HAD TO STEP ON IT FIRST
THE EMPLOYEE WAS STEPPING OFF OF A WORK PLATFORM & MISSED THE STEP. HE STARTED TO FALL & REACHED OUT TO CATCH HIMSELF WITH HIS RT. HAND. HE BROKE HIS RT. WRIST WHICH HAD PREVIOUSLY BEEN BROKEN IN A WORK RELATED ACCIDENT & WAS STILL IN A CAST.
Employee was climbing access ladder on a WA800 front end loader when he fell to the ground. Employee right foot slipped on second rung from cab platform and he lost his grip with right hand. He swung out to the left while left foot and left hand still on loader when he lost his grip and fell an estimated 8.5 feet to the ground landing on his right side.
EE cut right hand open when he slipped and fell walking to begin shift. the ee was crossing a wooden bridge from ee building to the stockyard and slipped on the wet/ icy bridge. When he braced against the fall, his hand struck the planks of the bridge and cut open. The ee was transported to MedFirst at 8 AM for treatment. required stitches.
WHIILE DISEMBARKING FROM RT SIDE OF LOADER, WELD ON PLATFORM BROKE. EE THOUGHT THE PLATFORM WAS COMING OFF AND JUMPED BACKWARDS LANDING WITH FRLL WEIGHT ON LEFT LEG.
EE was surveying area and slipped on ice while walking down a slope. Initial diagnosis was knee sprain. EE continued to have problems with knee. Later determination was ligament damage. Physical therapy was attempted to avoid surgery. EE had orthoscopic surgery on 5/24/05 and returned to full duty on 6/29/05.
Employee was accessing a clogged chute when he twisted his ankle.
Employee was exiting skid steer loader and twisted his ankle resulting in a chipped bone
Employee was walking on dredge when he slipped and hit his knee on a pipe.
WHILE GAINING ACCESS TO MACHINE, EE WAS ON STAIRS PULLING HIMSELF UP BY HANDRAILS. HIS FEET CAME OUT FROM UNDER HIM DUE TO OIL COVERED WITH DIRT ON TRACKPADS. BODY WEIGHT WAS HELD UP BY ARMS. HE FELT A PAIN FROM HIS RIGHT SHOULDER. THIS CASE WAS TREATED AS A FIRST AID CASE UNTIL 1/13/05. AT THAT TIME AN MRI SHOWED A TORN ROTATOR CUP. PHYSICIANS ARE RECOMMENDING SURGERY.
EMPLOYEE WAS GETTING OUT OF THE WATER TRUCK AND HE SLIPPED ON THE RUNNING BOARD.
EE STEPPED DOWN OFF OF A D8 CAT ONTO THE GROUND. AS HE WAS WALKING AWAY, THE EE STEPPED ONTO THE RIDGE OF DIRT LEFT BY THE WHEEL TRACKS AND ROLLED HIS FOOT OVER CAUSING HIM TO FALL TO THE GROUND.
Employee was walking around the end of the maintenance truck when he fell and cut himself on the belt knife blade that was open.
Ee was carrying and bucket of 89"s to blast holes when he slipped going down a 3' embankment twisting leg and hurting right knee.
LADDER WAS PLACED ON LOOSE MATERIAL & BECAME UNSTABLE CAUSING A FALL. REQUIRED SAFETY EQUIPMENT WAS WORN AT TIME OF ACCIDENT & CAME OFF DURRING FALL ALLOWING THE UNBOLTED GRILL OF THE TRACTOR THAT THE EE TRIED TO GRAB FOR TO COME OFF & IT STRUCK & CUT HIS HEAD.
Emp. reports he had gone under a handrail onto a 15" plank on the dredge middle deck with 1-1/2" hose to rinse sand out of chute that runs into #2 crusher. He pulled on hose to release slack. Hose fell between plank & sand tank pulling him to lower deck. Was not wearing fall protection.
EE was exiting screen and fell approx 36 inches and struck his arm on a small stone.320042820011-update returned to work full duty with no restrictions.(EE was inside the screen doing maintenance whent he job was finished other instructed EE to get out of the screen. On his way out of the screen EE lacerated his right arm just above the elbow on a sharp rock.
Employee turned his ankle while exiting a skid steer loader. Employee was taken to emergency room and treated for a sprain ankle and given three days light duty.
EMPLOYEE WAS TALKING WITH A VENDOR IN THE HALLWAY OF THE OFFICE. SHE TURNED TO RETURN TO HER DESK & TRIPPED OVER SOME BOXES THAT HAD BEEN STACKED IN THE HALL.
2 Non-mma customer haul truck were involved in a minor accident @ stockpile area. EE was taking photos when he fell injuring left foot.
Employee was Exiting a loader when he got his foot caught on the last step and fell to the ground
Employee was running to shut off a pump when he tripped and landed on his shoulder.
Employee was helping another employee carry a troughing roller when he miss stepped and twisted his lower back.
Driver was delivering sand to quarry. He dumped his load and was getting out of the cab when he missed the first step and fell approx 3' to the ground, resulting in a broken right leg.
Employee was re-fueling off road truck when he slipped off the berm twisting his left ankle.
Employee was stepping over a water line that was approximately 4" off the ground. Employee's right foot caught on the water line and he stumbled and while trying to catch himself with his left foot, he sprained his left ankle.
WHILE FIXING A SCREEN, HE STEPPED OVER SOME SCREENING MATERIAL AND BENT HIS KNEE THE WRONG WAY WHEN HIS FOOT SLIPPED.
Employee was checking the screen on top of the crushing plant. As he started back down the ladder, he did not have his eyes on path and his foot missed the top step of the ladder causing him to fall approximately 8 feet to the catwalk located on the next level below.

CO-WORKER WAS MOVING DREDGE, EE WAS IN KITCHEN OF VESSEL. INJURED EE STATES THAT HE STEPPED ON CHAIR TO RETREAVE GLASSES FROM THE TOP CABINET, LOST HIS BALANCE, FALLING INTO A WINDOW. BOTH OF HIS HANDS WENT THRU GLASS.
EMPLOYEE WAS ASSEMBLING A HYDRAULIC HOSE IN THE HOSE ROOM WHEN HE STEPPED ON A BAD SPOT IN THE FLOOR AND SPRIANED HIS KNEE.
EE WAS WALKING BETWEEN THE SAND PUMP AND SAND SCREW AND TRIPPED ON AN ELECTRICAL CONDUIT, WHEN THIS HAPPENED THE EE EXTENDED HIS LEFT ARM AND LANDED ON THE GROUND. HAD A SMALL FRACTURE TO LEFT ELBOW.
EMPLOYEE WAS GETTING OFF OF LOADER TO CK PORTABLE CR. LAST STEP PUSHED IN CAUSING HIM TO SLIP & TWIST HIS LEFT ANKLE. APPEARED TO BE OK. HAD TO SEE DR 2-3-04.
EMPLOYEE HAD JUST SERVICED A HAUL TRUCK. AS HE WAS COMING DOWN THE LADDER, BOTH FEET SLIPPED ON THE BOTTOM STEP. WHEN HIS FEET CAME IN CONTACT WITH THE GROUND IT JARRED HIS LOWER BACK.
EE WAS WALKING ACROSS YARD WHEN HE STEPPED ON SOME ICE TWISTING HIS ANKLE.
EE SLIPPED ON THE ICE AND FELL ON HIS LEG.
EE TURNED TO WALK AND TRIPPED OVER A HOSE REEL STRAINING HIS BACK.
EMPLOYEE WAS LIFTING A PIECE OF BELT MATERIAL WHEN HE TRIPPED AND STRAINED HIS BACK.
EMPLOYEE FELL OFF OF HIS HAUL TRUCK AND STRAINED HIS BACK.
EE WAS APPROACHING A PARKED CUSTOMER TRUCK TO WAKE THE DRIVER, WHEN HE SLIPPED AND FELL, HIS LEGS WENT UNDER THE RUNNING BOARDS OF THE TRUCK. HIS SHINS STRUCK THE TRUCK, CAUSING THE INJURY.
EE WAS GETTING OFF OF LOADER AT END OF SHIFT, WHEN HE SLIPPED AND FELL AGAINST A LARGE ROCK NEARBY.
EE WAS WALKING INTO EE LOCKER ROOM WHE HE SLIPPED AND FELL, STRAINING A MUSCLE IN HIS NECK.
EE GOT OUT OF TRUCK CAB TO CHECK THE DISTANCE ON THE BLIND SIDE OF TRUCK. WHILE WALKING ON HOOD OF TRUCK TO THE FAR SIDE, HE FELL OFF THE TRUCK HITTING THE GROUND.
EE WAS GETTING OFF OF HIS STOCKPILE TRUCK WHEN HE SLIPPED ON ICY GROUND & HIT HIS LEFT KNEE ON THE FUEL TANK STEP. THIS RESULTED IN A CONTUSION. ORIGINALLY THIS INCIDENT WAS THOUGHT TO BE A FIRST AID ONLY BUT HE HAS BEEN DOWNGRADED TO MEDICAL/RESTRICTED DUTY. 20 DAYS INTO THE RESTRICTION THIS EE WAS LAID OFF.
WHILE PERFORMING REPAIRS EE FELL AGAINST HAND- RAIL.
EE WAS ALLEDGEDLY CLIMBING DOWN FROM HIS LOADER FOR LUNCH. WHEN EE PUT WEIGHT ON HIS RT.KNEE ON THE GROUND HE EXPERIENCED PAIN. EE WAS TAKEN TO LOCAL DR. WHO DIAGNOSED A KNEE STRAIN THAT WAS AN AGGRAVATION OF AN AGE RELATED CONDITION. EE WAS GIVEN A CORTAZONE INJECTION AND PRESCRIPTION FOR ANTI-INFLAMATORIES.
