Miner was climbing down the ladder on a 35 ton haul truck. The ladder was broken and EE's foot slipped through the ladder while the ladder flipped up into the wheel well. EE fell backwards onto the ground 3 feet below. EE landed on EE's back with EE's leg stuck in the ladder. EE did not submit EE's injury report until 02/08/2022.

A truck driver/mine employee was on trailer supervising the loading of the trailer. EE took a few steps backward and fell off the trailer and landed on neck. EE sustained a fracture of the seventh cervical vertebra.

SLIPPED ON OIL ON FLOOR AND FELL ON HIS BUTT. FLOOR HAD OIL DRY ON IT AND IT WAS JUST SWEPT AND NEW OIL DRY WASN'T PUT DOWN.
EE WAS PRESSURE WASHING A LOADER AND SLIPPED AND HIT LOADER CUTTING OPEN RIGHT SIDE OF HEAD.
