Miner was climbing down the ladder on a 35 ton haul truck. The ladder was broken and EE's foot slipped through the ladder while the ladder flipped up into the wheel well. EE fell backwards onto the ground 3 feet below. EE landed on EE's back with EE's leg stuck in the ladder. EE did not submit EE's injury report until 02/08/2022.

Employee was leaving for the day when they slipped on ice in the parking lot and twisted a knee.

He was placed on company-ordered light duty. His pain continued; on 08/20/2007 he was sent to the clnic diagnosed with lumbar strain. meds and therapy. restricted duty.

Worker was climbing out of a loader when he slipped on the step causing him to fall and sprain his ankle.

EE was fueling her forklift and fell off the step. She twisted her right ankle and bruised the right knuckle on her ring finger.

HE WAS CATCHING A STONE AND HE STEPPED INOT A GROOVE IN THE CEMENT. THIS CAUSED HIS ANKLE TO TWIST.

SAID HE GOT OUT OF LOADER AND FELT A SNAP IN HIS RIGHT KNEE.

EE SAID HE SLIPPED ON THE ICE BACK IN JAN AND HE COULD NOT REMEMBER THE DAY. NOW HE HAS BEEN HAVING HEADACHES. HE WENT IN FOR A CATSCAN AND WAS OFF OF WORK FOR 1 WEEK AND HE ALSO HAD A 20 LB R ESTRICTIONS FOR 1 WEEK.

SLIPPED ON OIL ON FLOOR AND FELL ON HIS BUTT. FLOOR HAD OIL DRY ON IT AND IT WAS JUST SWEPT AND NEW OIL DRY WASN'T PUT DOWN.

EE WAS PRESSURE WASHING A LOADER AND SLIPPED AND HIT LOADER CUTTING OPEN RIGHT SIDE OF HEAD.
