As he approached the outside form, a wall tie that had not yet been welded to the rebar mat, caught his boot or pant leg. Causing him to trip and fall. He hit the form with his knee and his elbow landed on asphalt paving.
Employee was standing in flat bed of company truck & lifting up on a crate so forklift operator could get forks under load to remove it from truck bed. The wooden crate board he was grasping broke when lifted, causing employee to fall backward over the side of flatbed, catching his right foot temporarily under the load, before falling to the ground and bruising his right knee.
EE WAS DESCENDING A STAIRWAY ONTO A LANDING. HE RELEASED HIS HAND FROM THE HANDRAIL AND TWISTED HIS ANKLE ON THE LANDING.
EE WAS PULLING WIRE FOR C-404 BAGHOUSE SUPPLY FAN. WHEN STEPPING OFF THE LADDER, HIS FOOT SLIPPED AND HIS BODY WEIGHT CAME DOWN ON HIS TOES, JAMMING THEM. EE WORKED REMAINDER OF THE DAY AND THE FOLLOWING DAY. HOWEVER, HE WAS UNABLE TO WORK 2-17-02 DUE TO PAIN AND SEVERE SWELLING.
