Employee was dismounting from loader when boot caught on bolt causing employee to loose balance and fall.
Employee was walking into office trailer when employee slipped on icy step. Employee fell and lost consciousness.
Employee was walking across scales when ee caught boot on grating causing ee to fall and hit head on side of building.
Employee was exiting from skid steer when employee rolled ankle resulting in a sprained ankle.
At the end of shift employee was walking to truck to get a wrench and stepped on uneven ground and twisted right ankle.
Employee was on the screen tower using a hose to clear a plug up of hard pack on conveyor when his foot slipped on wet surface causing him to lose his balance, but not fall. His left knee was twisted as a result. He was treated for a ligament injury and was given a brace, crutches and work restrictions.
Employee was dismounting his loader when he twisted his knee. He was using three points of contact however the step was slippery. The employee was evaluated, treated for a strain to his right lower leg and released to modified duty.
Employee was dismounting from a 980G loader and missed the last step of the ladder, resulting in a fall to the ground. He was evaluated and diagnosed with a bruised muscle in his back.
Employee states he was looking for a piece of steel on the steel rack. The area was covered in snow, he plowed and shoveled an area around the rack. When leaving the area, the employee states he caught his right foot on a pallet and fell on his right knee. He fell onto a piece of steel causing a laceration to the right knee.
Employee claims he was walking with his employees at the end of their shift back to the employee trailer. He descended a bank from the quarry road to the rail road tracks; approximately a 2.5 foot drop between the two and he lost his footing. The path was covered in snow which was packed down from use.
Sprained right ankle. Caused by employee bouncing on foot to give momentum to climb ladder into water truck cab. Ladder had been previously modified at this employee's request to make access easier.
2" pipe laying on shop floor. Miner broke right wrist.
Employee states he twisted as he was exiting out of skid steer and felt pull in leg/hip area.
EE WAS IN THE PROCESS OF CHANGING THE EXTENSION CORD TO THE VIBRATOR WHEN HIS FEET SLIPPED AND HE GRABBED THE EDGE OF THE CHUTE TO PREVENT FALL. RESULTED IN LACERATION TO EE'S RIGHT HAND AND ABRASION TO RIGHT ARM.

EMPLOYEE WENT TO ENTER THE CAB OF HAUL TRUCK, MISSED DOOR HANDLE W/ HAND AND LOST BALANCE FALLING OFF OF THE TRUCK FEET FIRST. THE FALL WAS APPROX. 8' AND THE EMPLOYEE SPRAINED HIS ANKLE. THE EMPLOYEE WILL BE ON LIGHT DUTY FOR 7-10 DAYS.
FELL FROM LADDER WHILE CLIMBING TO CHECK SCREEN HEAD OIL LEVEL.

AS EMPLOYEE WAS CLIMBING TO CAB OF TRUCK, HE SLIPPED WHILE HOLDING ON THE THE HAND BARS AND WRENCHED HIS LEFT ELBOW.
WHILE EMPLOYEE WAS CARRYING A DIVERTER PLATE, HESLIPPED AND CUT HIS THUMB FROM THE EDGE OF THE PLATE.
WHLE EXITING BOBCAT WITH FORKLIFT APPROACH ON, HE SLIPPED ON TIRE & PUT LEFT ARM OUT TO CATCH HIMSELF FROM FALLING. NOTE: EE DID NOT REPRT THAT HE RECD MED TREATMENT UNTIL 7/13/00
WHILE MOVING THE WATER PUMP FROM ONE END OF THE SCALES TO THE OTHER, HE LOST HIS BALANCE AND STEPPED INTO A MANHOLE CUTTING HIS RIGHT LEG.
