The contract truck driver working for STM reported slipping off the last step of the truck and falling to the ground when in the process of trimming the load. EE reportedly put hands out to catch self. EE left the site after falling and sought medical attention.

Pulling debris from import loads employee stepped backward and lost footing. Employee used left hand/arm to stop fall resulting in a wrist fracture.

Employee suffered a right calf strain when he stumbled handling a hose to wash mud from his haul truck. He did not feel it needed medical attention until 9/24/2014. He was treated for the strain and given modified duties until 9/29/2014.

Employee suffered a left knee strain as he descended the top of an earthen berm. As he turned to walk down the berm, his right foot slipped on loose material, twisting his left knee. He was provided prescription meds and a knee brace.

Employee received a hypertextension injury to his left thumb while clearing spilled material adjacent to a shaker screen. The employee was using a shovel to clear rocks that had spilled from the 711 screen onto the work deck. His foot slipped on a rock causing him to slip and fall which resulted in the shovel handle jam into his left thumb.

Employee was working with others repairing a crusher. As the employee was walking on the catwalk, he stepped on an uneven surface and twisted his ankle, causing him to fall. Employee was diagnosed with an avulsion fracture and placed on modified duty.

Employee was entering the crusher hopper to begin replacing liners. Employee lost his footing on loose material lying on the belt and fell onto the liner wall striking it with his right shoulder.

Employee was checking baghouse #2. When he stepped off the slab, he sank in the mud which appeared to be hard ground. His right leg sank about 14". Employee started to twist and turn before he realized his footing was unstable.

While welding grizzly bars on the dredge the employee lost his footing falling into a sitting position and striking his left elbow on one of the bars.

As the employee was getting ready to step on the crusher he was working on, he stepped on a piece of angle iron that was tack welded on the liner. The angle liner broke off causing the employee to fall about three feet backwards onto the head of the crusher.

The employee was walking fast from the plant, past the C-2 Conveyor towards the S-2 Shaker. It was dark so he did not see the un-even ground. The employee tripped and fell landing on his right shoulder.

The employee was repairing a hole in Chute C-9 while using an extendable ladder. The legs on the ladder he was working on slipped out causing the employee to fall. He struck his leg on the tailguard.

The employee was removing some wood blocks from the back of a service truck. When he stepped away from the truck he tripped and fell. To catch his balance, the employee leaned back towards the truck and caught his left little finger on blocks from the bed of the service truck.

AFTER WORKING HIS REGULAR SHIFT, THE INJURED EMPLOYEE CLEANED UP AT HOME AND FOLLOWING A SHOWER,NOTICED PAIN IN HIS TORSO. HE SOUGHT MEDICAL ATTENTION THAT EVENING AND THE PHYSICIAN DISCOVERED THAT HE HAD SUFFERED A BROKEN RIB. AT PRESENT, IT IS UNKNOWN HOW THIS INJURY OCCURRED, HOWEVER HE BELIEVES HE MAY HAVE SLIPPED UPON ENTERING AND/OR EXITING THE FRONT END LOADER.

EMPLOYEE WAS CLIMBING DOWN THE LOADER LADDER AFTER LOWERING THE BUCKET TO THE GROUND. ABOUT HALF WAY DOWN, THE LADDER, HIS HAND SLIPPED OFF THE HANDRAIL WHICH CAUSED HIM TO FALL APPROX 4 FEET TO THE CONCRETE SLAB.
