Multiple employees were conducting maintenance on the Sandvik Crusher at Tower 1. Two employees were working on the platform approximately 3 feet above the ground surrounding the crusher, one employee lost footing due to loose material and fell. Plant management and safety were immediately notified, Core Health was contacted, and the employee went to a local clinic for monitoring.

EE was servicing a pump. While doing so, the EE slipped catching their leg on a piece of angle iron resulting in a laceration needing sutures.

EE arrived at the work site prior to the start of his shift. EE proceeded to head towards the mechanic shop EE parked his car in a location that had been plowed last week it was not the designated parking area. The EE got out of his car and proceeded to walk over an area of accumulated snow and ice. While doing so lost his footing and fell on his side.

Height of fall and rainy weather conditions.

While checking oil on loader stepped back and fell off elevator platform.

Employee was getting out of his service truck #59-152 when he slipped on the ice. He struck his back on the side of the truck.

CLIMBING UP LADDER OF 988F (37-108) SLIPPED ON LADDER AND FELT PAIN IN SHOULDER.

OPERATOR SLIPPED (DIDN'T FALL) ON AN ICY SPOT AND PULLED HIS LEFT HAMSTRING.

EMPLOYEE CLIMBED UP ON HAUL TRUCK TO CHECK FLUID LEVELS. HE SLIPPED ON THE HOOD AND LANDED ON HIS RIGHT KNEE.

EE WAS WALKING THROUGH THE GARAGE. HE TRIPPED OVER A JACKHAMMER AND FELL. HE LANDED IN SUCH A WAY AS TO FRACTURE HIS FINGER AND SPLIT THE SKIN REQUIRING STITCHES.

EMPLOYEE WAS ON A SIT DOWN CREEPER WORKING ON AN ENGINE, HE LOST HIS BALANCE. AS HE WAS FALLING HE HE GRABBED THE BODY OF THE TRUCK WITH HIS LEFT HAND. HE SUSTAINED AN INJURY TO HIS LEFT PINKY FINGER.

EMPLOYEE CLIMBED OUT OF THE CAB OF HIS TRUCK TO MOVE STEEL RODS THAT WERE IN THE WAY. AS HE WAS DOING THIS HE SLIPPED.

EMPLOYEE STATES THAT HE TWISTED HIS KNEE WALKINGON UNEVEN GROUND.

FELL FROM SECOND RUNG OFLADDER AND STRAINED THE MINER'S NECK.
