The miner was climbing a ladder and as ee was repositioning hand both feet slipped and ee fell to the ground. It had snowed the day before.

A haul truck driver felt what felt like a sting on left leg. When EE looked at it, it was swollen and fluid was oozing from it.

The miner was changing a faulty roller on a feed hopper. The roller slipped and fell back striking the miner on the side of the head.

THE MINER WAS STANDING ON A LEDGE APPROXIMATELY 1 FOOT HIGH. HE WAS TIGHTENING A BOLT ON THE SAND PRESS WHEN HE SLIPPED AND FELL BACK HITTING HIS HEAD. THE LEDGE WAS SLIPPERY.

The operator saw a pipe on the ground. He got out of his loader turned around and as he stepped down missed the first step. The operator fell to the ground fracturing his femur.

The employee was cleaning off build up on a concrete block under a conveyor. He slipped and struck his hand between the framework and the pry bar he was using injury his middle and index finger.

The worker was cutting conveyor belting to fit under a conveyor to deflect material off the cat walk. As the worker was cutting with a box cutter, he slipped and struck his other hand (left) cutting the fifth digit.

The employee was on a ladder tightening the bolts to a counter weight bracket. He thought he was going to fall and held tight to the ladder which must have tweaked his back by holding on so tight. He had fall protection on if he would have fallen.

The employee was pushing on a frozen underside on a stacker belt, he slipped on some ice and fell hitting his mouth on the metal framework of the stacker. The employee has two broken teeth and a lacerated lower lip.
