Plant Manager was reversing the rotation(polarity) of the sand screw motor at the electrical panel which created an arc flash in the panel

Worker was cutting a rubber boot on the wash plant using a razor blade in right hand. While cutting, the blade slipped and cut left hand ring finger.

The worker slipped and fell on tailbone. The conditions were very slick due to ice and snowpack.

Mine worker was picking chunks of mud from the feed conveyor belt. Worker felt a strain and soreness in upper wrist/forearm area.

The miner was working with another worker to move a 400 lbs. roll of conveyor belting and hurt back.

The right side ride control went out on the loader causing the seat to bounce.

The miner was picking mud at a mud picking station. As the miner was reaching to throw a chunk of mud from the conveyor, ee strained back.

THE MINER WAS PUMPING WATER INTO A WATER TRUCK. THE CONNECTION BLEW APART AND STRUCK THE MINER ON THE LEG.

The employee was taking apart a conveyor stand for transfer when the support fell on his foot breaking a bone in his right 5th digit. The employee was wearing steel toed shoes.

THE EMPLOYEE WAS HELPING TO REMOVE A SCREEN FROM THEIR CRUSHER AND IT SLIPPED CATCHING HIS FINGER AND BREAKING IT.

EE WAS BUILDING BERMS ON A STOCKPILE. HE SAYS HE HAD HIS LOADER BUCKET 1/2 FULL WHEN HE DROVE BACK OVER A DIVOT HE CREATED CAUSING THE BUCKET TO HIT THE GROUND WHICH HURT HIS NECK. THE INCIDEN T OCCURRED ON MONDAY & WAS NOT REPORTED UNTIL FRIDAY.

THE EE WAS REMOVING A TRAILER HITCH FROM A PICKUP WHEN THE HITCH AND TRAILER ROLLED FORWARD PINNING THE EMPLOYEE'S FINGER AGAINST THE PICKUP.
