Fall from scaffolds, walkways, platforms

No adverse conditions. The fall site is a ladder and catwalk next to a log washer. This washplant is located inside a building. Environmental conditions are not a factor.

the miner was changing a bearing on a belt press. the miner said the injury occurred when lifting the old bearing off and twisting at the same time.

Fall to the walkway or working surface

The miner was climbing a ladder and as ee was repositioning hand both feet slipped and ee fell to the ground. It had snowed the day before.

THE MINER WAS STANDING ON A LEDGE APPROXIMATELY 1 FOOT HIGH. HE WAS TIGHTENING A BOLT ON THE SAND PRESS WHEN HE SLIPPED AND FELL BACK HITTING HIS HEAD. THE LEDGE WAS SLIPPERY.

The employee was cleaning off build up on a concrete block under a conveyor. He slipped and struck his hand between the framework and the pry bar he was using injury his middle and index finger.

The employee was prying frozen material from the feed hopper. A section came loose and the employee fell back. He caught himself and hurt his back in doing so.

The miner was putting back together a pump after changing the seal and it slipped causing the pump to fall and smashing his right index finger

Fall from machine

The employee fell between his excavator and a retaining wall at the fueling area because of muddy conditions. He bruised his hip and knee and tore the muscle in his bicep.

Struck against stationary object

The employee was actively wrenching on a bolt while repairing the crusher, when the wrench slipped and the employee hit his elbow. The employee said he would be alright and continued working. On Friday, the employee complained again about his elbow, but did not want to see anyone. On Saturday, the employee complained again and he went to the emergency room.

The crew was removing screens from a screen deck. This worker had a screen that was stuck and when he pulled hard the screen came loose, causing the screen to strike his chin. The chin had an abrasion and was swollen. He was taken to the doctor and was released to full duty.

Worker was walking from one side of mine to the other when the wind blew a foreign object into his eye. Was was wearing safety glasses. Worker went to urgent care but the extraction was unsuccessful so they referred him to the ER.

The employee was rotating gears to change timing and pinched his fingers between the bar and the frame. At first, the employee decided he was okay but later decided to see a doctor.

The employee was working in a washplant where there was welding being conduted by another employee. The flash must have bounced off the walls in the washplant and flash burned his eyes.

The employee was exiting a skidloader and his foot got caught and he fell sideways hurting his foot and maybe his toe.

Employee was pulling a shaft from conveyor belt. Pulled his right arm and shoulder. Diagnosis of right shoulder pain and right shoulder AC strain.
