The miner was cutting belting with a box cutter when the knife slipped and they cut their leg.

Fall from machine

The operator saw a pipe on the ground. He got out of his loader turned around and as he stepped down missed the first step. The operator fell to the ground fracturing his femur.

The worker cutting conveyor belting with a knife. He used a lot of force, the knife slipped and cut his left thumb through his glove.

Struck against a moving object

The employee says he had tingling going down his leg in the afternoon while he was operating a loader in the pit. Didn't report anything to his supervisor except on a voice mail late at night.

Struck by flying object

The employee was greasing a conveyor grease line when a rock bounced off and struck the employee in the face knocking out a tooth and loosening two others.

The employee was entering his skidster when he slipped and when he tried to catch himself, he hit the corner of the frame with his hand/wrist.

Fall from machine

Employee was doing maintenance work on the conveyor. When he stepped down, he fell to the ground and cut his face, requiring three stitches near his eye.

Rubbed or abraded

The employee was kneeling in front of a tail pulley he was doing maintenance on and when he got up, he felt pain in his right knee. The knee became swollen as well.

The employee was helping to grease a haul truck. He was holding a brace pin up as the bed of the truck was coming down and smashed his hand between the two.

A CONVEYER WAS BEING WORKED ON AND WHEN THE EMPLOYEE BRUSHED AGAINST THE ELECTRICAL CAUSING SPARKING AND POPPING WHICH CREATED A BURN ON THE EMPLOYEE'S ARM. A REVIEW OF THE LOCKOUT REVEALED THAT SEVERAL STITCHES WERE NOT ACCURATE. THEY HAVE CORRECTED SINCE.

AN 8 FT POLE IS USED TO BREAK UP TOP SOIL INSIDE A FEED HOPPER FROM AN ELEVATED CATWALK. THE EE IS NOT USED TO THE ACTIVITY AND DEVELOPED A MINOR STRAIN TO THE R WRIST.
