Plant Manager was reversing the rotation(polarity) of the sand screw motor at the electrical panel which created an arc flash in the panel

Miner was connecting pipe which was suspended from forklift to a fixed flange, as miner leaned down to connect the flanges, forklift boom moved abruptly, pushing the miner with the boom, causing the miner to fall forward & down into a hyperextended position. Miner was evaluated by physician and released with no restrictions. Miner was prescribed 800 mg Ibuprofen for contusion.

Miner was asked to tighten the clamp rail bolts on the outside of shaker screen. As EE turned, EE immediately ran into the handle of a ball valve at the end of a spray bar striking EE in the left eye. EE suffered a cut/laceration to the bottom of EE's eyelid. EE was released from the ER with eye drops & a follow up with a ophthalmologist.

The miner was cutting a zip tie wrap using a razor blade knife. While cutting, the blade slipped from the zip tie and cut the miners thumb. The cut required 4 sutures.

Two workers were moving pit pumps. The flange's on the poly pipe had to be removed. The fourth bolt would not come off, and the pipe was being held up by a chain that was attached to the excavator. EE used a torch to remove the bolt. When it came off, the poly pipe struck EE in the forehead.
