EE was walking as EE was stepping across the rail. The EE slipped and fell. At which time EE's right foot went under the bridle bar.
A roof fall occurred at 63 crosscut on 7B track. The fall was 4'high, 14' wide and 35' long. The area has been cleaned and re-supported.

A roof fall was discovered at approximately 6:14am. On the 1 Left B section belt. The roof fall is approximately 8' high, 16'wide, by 70' long
A roof fall was discovered in J Entry of the North A Belt area between crosscuts 3 and 4. The fall is approximately 8' high, 15' wide and 100' long. All accesses were dangered off.
EE was roof bolting a corner. As EE was retracting EE's side it caught the roof. Upon catching the roof it caused the slide out on the canopy to gap open EE caught EE's fingers in the gap.
At approximately 12:30 pm A supply man discovered a roof fall on the mantrip bottom.
Hopedale Mine elevator was down due to electrical problems. The elevator was repaired and placed back in service at 8:15 pm on 7/18/2025.
A roof fall was discovered on the East A Track at crosscut No. 9. The fall was cleaned and rebolted.
EE was removing stone that had fallen on the continuous miner. EE had both hands on a stone when their right hand slipped off causing the stone to fall. Which caused the stone to strike their left hand to strike the shovel.
EE was putting rub rail back on the continuous miner. EE caught EE's finger between the rail and the c/m.
At 11:20pm a call was made outside saying a roof fall was discovered on East A belt. At 11:22pm MSHA was notified. Upon investigation of the roof fall it was determined that the fall did not meet the criteria for reportable accident. Roof stone had fallen out from around the roof bolts. The area was approximately 14 feet long x 8 feet wide x 3.5 feet high.
What was originally thought to be a roof fall was actually a non-reportable occurrence. The immediate roof had fallen but was not above anchorage. The area was the track entry on 7 Seam A track at 55 crosscut. The area was cleaned and additional bolts installed.
A roof fall was discovered in the A Entry of the 7B belt line. The fall is approximately 16' wide, 8' high and 60' long. The fall has been posted and dangered. The fall will not need to be cleaned up.
EE was sitting on a 2 man johnson parked beside the miner. 2 other EE's was pulling the miner cable outby. They dropped the wrong loop of cable as they pulled the cable caught the johnson. The other EE that was on the johnson tried to get off it. EE was caught between the johnson and miner.
A unplanned roof fall was discovered at 7:10 am on the North A belt at crosscut 9. The fall was dangered off and posted. Rehab will be performed.
Weekly examiner encountered a roof fall in the primary escapeway "Intake" Located at 7 seam B at 35+11. Horizontal shifting. Approximate size 40 feet long, 17 feet wide and 9 feet high
Weekly examiner encountered a roof fall in the intake. Located at 43+67 F entry 7 seam B. Fall approx.18 feet wide x 18 feet high length is unknown due to an older roof fall in that area.
A fully trained miner was checking oil on a D-11 Dozer when EE slipped on the track of the dozer and landed on their shoulder. The injury was not the result of any compliance issues nor were any protective guards involved.
While checking the car buffer and counterweight buffer. Both buffers failed to work properly.
A fully trained miner was welding in a boom truck when the boom of the truck caught fire. The IE jumped from the bucket and the lanyard burnt in two causing the employee to fall to the ground. The employee suffered a broken pelvis.

EE was traveling to the slope bottom. They encountered a roof fall in North A track entry. Weathering was the cause of the roof fall.

EE was running a shuttle car, came off a ledge, hit tailbone on the seat
EE was helping change a bearing on a tailpiece. Another EE struck a chisel which flew 5 feet striking the EE right side of EE's face and right eye.
Pre shift examiner discovered a roof fall in the 7 seam B track entry. Fall was approximately 5-6 feet high, 14-15 feet wide X 35-40 feet long.
While EE stepped out of the shuttle car EE twisted their left ankle.

While moving C/M in cut, rock fell from roof striking lower right leg.

AT approximately 10:50 pm discovered a roof fall had occurred at 49+40 on 7B track entry . Fall was approximately 14 feet wide x 22 feet long.
EE was cutting the jacket on miner cable. Second EE had pliers on the jacket pulling it. First EE slipped with knife cutting second EE's fingers.
EE was walking down the tram road stepped in a hole felt pop in knee.
A water line burst underground. The situation was not discovered until Monday morning approximately 8:30am. The water line break caused the track and intake entries to flood stopping travel. The water is located 7 seam B from 23 to 28 x-cut and from 33 to 35 x-cut
Weekly examiner was making the weekly run. The examiner encountered a roof fall. The roof fall was at 19+00 in 7 seam B intake. The fall measured approximately 7 feet high, 18 feet high, by 60 feet long.
EE was putting crib blocks under the feeder. As EE was shoving a crib block underneath the feeder EE struck EE's left thumb. Which lacerated EE's thumb.
During the weekly exam the examiner encountered a roof fall in the return air course. The fall was at 42 x-cut 7A return. Fall measures approximately 60 feet long x 17 feet wide x 10 feet high. The fall had stack rock strata.
While traveling 5Left weekly air course, A roof fall was encountered just inby spad # 20+41 in F entry, measuring approximately 50' long by 17' wide by 7,high.
At 3:40am a roof fall was encountered on the 1 south section. Fall was inby the tailpiece. Location of the fall A entry @ 0+59 to 1+20. Fall is approximately 60 feet long,18 feet wide,by 12 feet high.
EE was plugging charger cable into a battery scoop. EE felt a pop and burning sensation in right elbow.
EE was roof bolting over loose roof material that had fallen. EE was lowering the boom on the bolter when EE's left knee slid under the boom striking EE's left knee.
EE was roof bolting. As EE reached for the drill a rock fell striking EE's right forearm. The rock was approximately 18 inches by 18 inches by 3 inches thick. The rock had sharp edges lacerating EE's arm. EE received 6 stitches.
EE was stepping out of EE's shuttle car. EE stepped on a rock with EE's left foot. Felt a pop and pain in EE's ankle.
EE was roof bolting. EE had installed 2 pattern bolts in the row. EE felt the roof needed an additional bolt in that row. EE swung over to EE's rib to drill the bolt hole. As the EE was putting EE's drill steel in EE's chuck. A kettle bottom fell from the roof striking EE's left hand.
Weekly examiner found roof fall at 1:55 pm in 1 Right section at 2+00 in F entry. Approximately 15 feet wide 20 feet long height is above the anchorage.
EE was in the process of applying Block bond on when some of it splattered into EE's left eye
EE was roof bolting. A piece of stone fell from the roof. Striking EE on right neck, shoulder, and arm.
