EE stepped out of a man basket and rolled EE's ankle after stepping on a rock. Went to Concentra, no lost time or restrictions, but was given crutches and a brace. Returned to work the following day.

Showing all 39I was pulling frozen pipe out of the ground or pulling on the hose while loading. (repetitive motions) and I felt a slight discomfort in my forearm. The next day I noticed to be much sorer and more fatigued than normal in my arm and after the shift I noticed a lump/ball on my forearm the size of almost two gumballs.
Miner was placing scrap in recently emptied dumpster while wearing hand protection when EE smashed their right thumb. EE's fingernail became loose and EE's thumb looked split.
A tail pulley guard was being removed to perform maintenance. The guard was stuck in place due to frozen dirt. The team removed what they could of the frozen material using force to remove the guard the rest of the way. As the guard became free, EE's thumb was caught between the guard and the frame. There was reason to believe EE may have broken EEs left thumb.
Miner pulled shoulder while pulling on large wrench to loosen bolts on cone crusher for maintenance.
Employee was changing receptable when EE contacted an energized receptacle resulting in a shock.
Miner was operating a skid steer when the pull bar became stuck, when miner tried to correct the pull bar it came loose and hit against the window breaking it. Miner tried to stop the pull bar from hitting the window with EE's hand and the weight of the bar injured EE's hand.
EE was using a 4 inch knife to open package of tools when the knife slipped, striking EE's left hand.
While installing a new starter on an excavator, the miner who had been in a kneeling position went to stand up to align the part into position on the excavator. While standing up the miner felt a pop in EE's left knee.
Miner pinched abdomen between a conveyor and a support board while repairing clips on the conveyor.
Miner was holding washers in place to be welded and was not wearing a welding helmet for the duration of work, approximately 25 minutes. The witness was roughly 3 feet away and was welding their own washers. A welding helmet was available and the miner was trained on the importance of wearing it.
EE was exiting work truck. When EE stepped onto ground, EE felt knee pop and give out.
EE was lifting oak block to block up crane when a large splinter lodged into left hand on palm below index finger. EE removed what they thought was the entire splinter. Next day, their hand had become swollen, red and painful.
EE received annual hearing test which noted Standard Threshold shift in right ear. EE is a plant operator who operates primary plant from control booth. EE is being scheduled for another hearing test with medical provider.
EE was grinding metal parts in garage. When completed, EE was opening/lifting garage door when EE felt immediate pain in left shoulder.
EE was installing new set of 48" idlers on conveyor. Bar in the idler slipped and came down and caught EE's thumb between idler and steel frame of conveyor.
Employee was removing last bolt holding hub assembly on right rear of loader. Blocks holding oscillating axle shifted causing axle to kick upward crushing pointer and middle finger on right hand between 3/4" air impact gun and frame of loader.
Employee was using a large bar to pry a rock out of a conveyor. While doing so employee's right hand struck a sharp object causing a contusion and laceration.
EE suffered a strain to the right shoulder while driving the truck on rough terrain.
Removing broken drill steel from machine. Each steel weighs about 100 lbs. Loaded 3 steel drills on truck. Felt pain by end of day.
Blasted rock was ejected from the blast. 3 buildings and 8 vehicles were hit. Another vehicle with 2 occupants was also hit. They were taken to hospital. injured #1 was released that day. The other was released the next day.

While moving a stone a second stone fell on top of first stone cutting right hand thumb.
Taking roll crusher frame apart, removing plates, loosened bolt came off pinching finger between socket and plate.
Clamp slipped on plate, which fell on leg and sprained ankle. Plate was being welded in body of haul truck.
Fall from height. Fractured ribs, abrasions and hurt back.
Employee was disconnecting lifting equipment from a load that was sitting on blocks, cable and shackle were slack, blocks kicked out from under the load, causing the cable to tighten, catching the left ring finger betweeen shackle and cable.
EE was attempting to remove a shaft with an eight pound sledge hammer, when the hammer slipped and struck his left pointer finger
Ee was installing a chute on a return belt. He was using a pry bar to align the bolt holes. He turned to speak with another ee and felt pain in his chest. He reported the incident and was directed to seek medical attention.
Employee stepped on a stone to get onto excavator tracks to grease machine. He fell off the rock and fell straight down 15+ feet, rolled 10 feet more, came to stop. Other employee found him, brought him to level ground, ambulance took him to the hospital.
Employee was removing frost from around track idlers on Akerman excavator with a hammer & chisel. A glancing blow off chisel head struck his middle finger on left hand against track frame, resulting in a deep cut & bruises to his finger.
Employee stepped off excavator and ankle rolled on uneven ground causing a sprain.
Employee was stepping out of the service truck and fell, injuring his knee.
One employee was removing clamp from spring bar on screen tower. He dropped one-half of the clamp. The clamp hit the catwalk and fell through hitting two employees on the head.
CLIMBING ONTO DRILL STRUCK SHIN ON RUNG.
EE WAS CLIMBING LADDER TO DRILL WHEN SLIPPED AND FELL BACKWARDS OFF LADDER.
A HOPPER GRATE RAIL BECAME DISLODGED. EE ATTEMPTED TO PUT BACK IN PLACE AND HIS FINGER WAS PINCHED AS IT SLID BACK INTO PLACE.
THERE WAS A LOG JAM OF ROCKS ON TOP OF THE GRATEOVER THE HOPPER THAT FEEDS THE PLANT. WHILE ATTEMPTING TO DISLODGE SOME OF THEM, A STONE FELL THROUGH THE GRATE BENDING HIS RIGHT FOOT OVER TH E RAIL. A SPRAINED FOOT WAS THE RESULT.
EMPLOYEE WAS CLEANING THE WINDOWS ON THE EUC WHEN HE LOST HIS BALANCE AND JUMPED.
