Flash burns (welding)

Miner was holding washers in place to be welded and was not wearing a welding helmet for the duration of work, approximately 25 minutes. The witness was roughly 3 feet away and was welding their own washers. A welding helmet was available and the miner was trained on the importance of wearing it.

Ee was installing a chute on a return belt. He was using a pry bar to align the bolt holes. He turned to speak with another ee and felt pain in his chest. He reported the incident and was directed to seek medical attention.

Employee stepped on a stone to get onto excavator tracks to grease machine. He fell off the rock and fell straight down 15+ feet, rolled 10 feet more, came to stop. Other employee found him, brought him to level ground, ambulance took him to the hospital.

Employee was removing frost from around track idlers on Akerman excavator with a hammer & chisel. A glancing blow off chisel head struck his middle finger on left hand against track frame, resulting in a deep cut & bruises to his finger.

Employee stepped off excavator and ankle rolled on uneven ground causing a sprain.

Struck by falling object

One employee was removing clamp from spring bar on screen tower. He dropped one-half of the clamp. The clamp hit the catwalk and fell through hitting two employees on the head.

THERE WAS A LOG JAM OF ROCKS ON TOP OF THE GRATEOVER THE HOPPER THAT FEEDS THE PLANT. WHILE ATTEMPTING TO DISLODGE SOME OF THEM, A STONE FELL THROUGH THE GRATE BENDING HIS RIGHT FOOT OVER TH E RAIL. A SPRAINED FOOT WAS THE RESULT.
