EE had driven wedges between bucket and cutting edge while torching off bolts. The cutting edge sprang forward and caught the ee in the knee. This caused bruising and swelling in his knee later that evening.

The impact crusher had stalled leaving rock lodged inside. Employee's were rolling boulder out of impactor when employee's hand was caught cutting his finger. The two employees were not communicating together and the rock slid onto the employee's hand causing the injury.

Employee was lifting drill steel to put into changer when his wrist popped. He had hurt his wrist the privious year while working at home. Employee went home that evening and came to work the next morning & said he experienced more pain. After a doctor's visit it was determined to be a sprain.

Employee was holding a cylinder with his left hand. While trying to set it in place, cylinder hung up and became dislodged. This caused the cut to his hand. The cut to his hand required 6 stitches.

He was grinding a piece of metal to weld. The wind blew a piece of metal behind his safety glasses. The metal couldn't be flushed from his eye.

The wind was blowing hard while they were changing screens. Some dust blew into employee's eye. Later that evening, his eye started bothering him more so he went to the hospital to have the dust removed.

Employee stated that he has been wearing proper hearing protection while working around equipment. Employee also stated that he has started to wear hearing protection while working at home. He is unsure why there was a change in his hearing.

Employee stated that he skipped the 2007 hearing test so two years went by between tests. He stated that he had been wearing all hearing protection needed while running equipment. He believes his age has played a part in the hearing loss.

Employee stated that on the day of the hearing test he had a head cold which may have affected the results. He has been wearing proper hearing protection at work & does not believe he has been over-exposed at work.

Employee works in the office and does not run equipment. He stated that he wears proper hearing protection when needed. He believes that the hearing loss is due to hereditary causes because other members of his family have hearing loss.

Employee was lifting a cover weighing approx. 40lbx & his back was strained.

Changing the bolts on cutting edge using an inpact wrench, bolt spun and cut 2 fingers.

Changing screen panels on screen deck, removing chute and slipped and fractured finger.

Getting out of loader left foot hit ground first, landed on rock twisted ankle.

Was clearing out an chute with a large pry bar and struck a peice of metal in 2nd and 3rd between the two. got 3 stitches.

USING A LADDER TO PUT A BOLT IN AN IMPACTOR II, EMPLOYEE STRAINED HIS BACK.

Was cutting crown bar rubber for an screen deck and slipped and cut top of 2 fingers.

EE WAS PULLING LARGE ELECTRICAL WIRES FOR THE NEW CONTROL HOUSE. HE WAS TRYING TO BEND THE WIRE AND HEARD A POP IN HIS RIGHT HAND.

Ok first was into he skid-steer cleaning up the plant, came into the parts trailer to eat lunch and hurt his back in the process of sitting down.

CHANGING GRATES IN HAMMERMILL AND SMASHED LEFT THUMB.

WHILE CUTTING A TREE LIMB WITH A 20" BAR CHAIN SAW, EE ACCIDENTALLY CUT HIS LEFT BIG TOE, WHICH REQUIRED 7 STITCHES.

EE WAS PULLING ON IMPACT ROTOR ON CRUSHER TO CHECK WEAR ON IMPACT HAMMERS HE FELT A PAIN IN HIS LOWER BACK WHEN HE PULLED ON ROTOR TO ROTATE IT.

EMPLOYEE ALLEGES HE WAS HOOKING WELDER TO PICK UP. BENT DOWN TO LIFT UP HITCH AND FELT SHARP PAIN IN LOWER BACK.

EMPLOYEE WAS WALKING ACROSS ICE COVERED PIT FLOOR AND SLIPPED ON THE ICE.
