Over-exertion (Not Elsewhere Classified)

Employee was helping with maintenance on the Norberg cone crusher. EE went to climb down the step ladder, the ladder shifted while still holding onto the cone with EE's left hand. Employee felt a pop in EE's shoulder. Employee had surgery on 02/19/2026 and was given restrictions making this reportable.

Employee was using carpenters knife to cut grease line when the knife slipped off the hose and cut left thumb knuckle. Did not require stitches, was placed in a splint to immobilize due to the location of laceration.

Employee was adjusting the mirror on the dump truck when the employee's foot slipped off the step and EE struck right elbow on the storage box attached to the inside door panel. Incident became reportable on 9/10/2024 when EE went to the doctor and was issued prescription medications.

Struck by flying object

Employee was using a wire wheel on a rusted piece of metal when employee felt something go into eye, underneath safety glasses. Employee flushed eye but still felt something in there. Went to clinic, where they removed a piece of metal and issued prescription antibiotics.

Employee was cutting flashing from under a rock box with a folding carpenters knife and the knife folded on employee and cut palm

Employee got into dump truck to check fire extinguisher and stepped out of truck back on ground and felt lower back pinch. About 30 min. later EE shoveled off some build up on a screening unit and felt back get worse.

The employee was placing a new section of culvert pipe on a existing culvert pipe. While placing the new sleeve the edge of the new pipe cut the employee on tip of the hand on knuckles.

The employee was climbing down a ladder when he got to the bottom and turned he felt a pop in his knee. Took him to the doctor and they thought he sprained it. After awhile it did not get better so we had a MRI down which concluded he had a tear in the meniscus on 6/10/15 which will required surgery.

EE had driven wedges between bucket and cutting edge while torching off bolts. The cutting edge sprang forward and caught the ee in the knee. This caused bruising and swelling in his knee later that evening.

Employee was holding a cylinder with his left hand. While trying to set it in place, cylinder hung up and became dislodged. This caused the cut to his hand. The cut to his hand required 6 stitches.

Employee stated that on the day of the hearing test he had a head cold which may have affected the results. He has been wearing proper hearing protection at work & does not believe he has been over-exposed at work.
