Over-exertion (Not Elsewhere Classified)

Employee straightened after stemming a hole and felt a sharp pain in EE's right pectoral muscle that didn't go away for the rest of the day. Commented on it to 2 other EE's. Dull ache with certain movements triggering more intense pain. Worsened over the weekend.

Caught in, under or between a moving and a stationary object

Employee was adjusting impact crusher and one employee was raising and lowering cylinder and other employee was turning nut. When the one employee turned the nut the other employee pushed lever to lower cylinder, the other employee stuck hand back in quickly to re adjust and smashed two finger tips

Fall to the walkway or working surface

Employee walked up a bank to spray paint boundary line and turned to walked back down and feet came out from underneath EE and jarred EE's neck.

While climbing down off the machine the employee missed the last step of a two step ladder and turn and hit knee on the out rigger of the truck.

The employee was dislodging a rock that was hung in a conveyor and a piece came back and struck above left eye above safety glasses.

Caught in, under or between a moving and a stationary object

The EE was in the process of locking up site. He walked over to the pit area and had to close the sliding gate and lock it. While doing so the EE pinched his two fingers between two fixed pieces of metal causing a small cut on 2 fingers. The EE received stitches at the local hospital.

Employee stated that he skipped the 2007 hearing test so two years went by between tests. He stated that he had been wearing all hearing protection needed while running equipment. He believes his age has played a part in the hearing loss.
