While carrying screens for switching out on dewatering screw, EE felt discomfort in groin section. EE went to hospital and was diagnosed with hernia-type injury. Surgery was performed and EE was released next day.

While installing dredge handrail EE slipped on sand, a portion of handrail struck EE's neck, creating a minor laceration. Laceration was bandaged at local clinic.

Employee was repositioning a gravel dredge with a manual winch. A wind gust caught the dredge; the winch was in freewheel, the rotating handle struck the employee on the left forearm, lacerating the skin

Individual reported for duty on 6/6. He stated that he did not feel well. Supervisor sent for him medical treatment; no symptoms noted. Worker sent home for the day. He returned on 6/7; he felt worse. Further examination found a mild stroke prior 6/6. Individual is currently hospitalized. Report is filed as information only per MSHA-PEIR/Denver.

Supervisor had last contacted worker at 1:15 PM. Dredge operator called supervisor at 3:15, notifying him that he could not contact worker. Supervisor found worker unresponsive at the controls of the CAT972G loader he was operating. Loader was upright with bucket in pile. Worker transported to local hospital. Diagnosed as suffering from a stroke. No other injuries or damage.

Employee was grinding rivets on a belt splice, high wind condtions, outdoors. Wind blew small metal fragment into right eye. Employee washed out eye and continued work. Eye remained irritated. Sought medical attention on 8/4/9

Worker was exiting a shop trailer at the end of the shift. Access was by a manufactured stairway system with handrails on both sides. Worker missed first step and fell approximately 4 feet to the ground (packed sand). Worker landed on his left shoulder. Experienced severe pain the following morning. Orthopaedic consultation scheduled for 4/6/9.

Worker was cleaning a crusher interior. While dismounting, he slipped and did the "splits" while recovering. This hyper-extended his right groin muscle.

Worker had removed access panels to dredge control apparatus under the control house. He took a break to warm up. He fell into the access hole while reentering the repair area. He worked the remainder of his shift, by his choice. He did not seek medical attention until the next day.

Worker was performing servicing operations on a portable crusher. Work requires considerable bending at the waist. Upon straightening from a flexed position, worker noticed sharp pain at lower back and down left leg. Worker has experienced pinched nerve in this area in the past.

Injured worker had climber the ladder to clean a grizzly screen. As he stepped onto the platform, he struck his right knee against the guard rail. He continued to work his shift. His knee was swollen the next morning. He sought medical attention on 9/20.

Worker was helping install sluice pipe. He is primarily a trucker, and rarely performs manual labor for an entire shift. At the end of the shift, he complained of back pain. He sought chiropractic care on 5/21/04. He did not visit an MD until 6/14/04. Injured worker has lower back pain. He has been transferred to sedentary work at his normal wage.

EE WAS PART OF A 2 MAN TEAM LIFTING A SAND PUMP (~150 LB). EE USED A TWISTING LIFT, INJURING HIS LOWER BACK. HE MENTIONED INCIDENT TO HIS CO-WORKER, HE DID NOT SEEK MEDICAL ATTN. UNTIL 12/20/0 3. HE DID NOT REPORT INCIDENT UNTIL 01/02/04.

EE WAS ASSISTING IN CHANGING THE 12" FLEX COUPLING ON THE DREDGE SUCTION . THE SAFETEY CABLE BROKE THE COUPLING STRUCK HIM IN THE CHEST. HE LANDED ON HIS LEFT KNEE. ( HE HAD PREVIOUS SURGERY O N HIS KNEE PRE-DATING HIS EMPLOYMENT.
