EE was was working on the plant. EE was applying pressure to a prybar when EE's hands slipped off and EE's right little finger was pinched by the bar, resulting in a fracture to the tip of EE's right pinky finger.

A miner was checking fluids on a piece of equipment when EE lost EE's footing and ended up falling onto the ground. The fall resulted in EE fracturing EE's right wrist.

While adjusting conveyor belt tracking, EE was using hammer to strike adjustable return roller bracket. EE missed hitting bracket causing his right arm to be drawn into belt & he became wedged between belt & roller.
Employee was assisting in replacing a bolt on primary jaw crusher. While striking a slugging wrench with a sledge hammer, employee's hand struct ajoining metal rail causing a fracture of the tip of right thumb. Employee didn't go to clinic until two days after accident.
WHILE REPLACING A SCREEN IN A SAND SCREEN DECK, SCREEN SLIPPED AND CAUSED LACERATION OF TWO FINGERS ON RIGHT HAND (SUTURES REQUIRED).

STANDING ON THE FEED BOX OF A CRUSHER, 15' OFF THE GROUND, EE WAS REACHING ABOVE HIS HEAD TIGHTENING 2 BOLTS WITH A WRENCH. THE WRENCH SLIPPED OFF THE BOLT CAUSING EE TO FALL BACKWARD TO THE G ROUND.
