STANDING ON THE FEED BOX OF A CRUSHER, 15' OFF THE GROUND, EE WAS REACHING ABOVE HIS HEAD TIGHTENING 2 BOLTS WITH A WRENCH. THE WRENCH SLIPPED OFF THE BOLT CAUSING EE TO FALL BACKWARD TO THE G ROUND.
