EE was helping a delivery driver offload material. During that process, EE lost EE's grip and a piece of steel fell onto EE's left upper thigh resulting in a contusion.

A miner was checking fluids on a piece of equipment when EE lost EE's footing and ended up falling onto the ground. The fall resulted in EE fracturing EE's right wrist.

While cutting rubber belting material with utility knife, employee lacerated left hand. Employee received 3 sutures and was released to RTW-no restrictions.

While cutting/splicing conveyor belting, utility knife slipped causing laceration on right thigh. Employee required three sutures.

While adjusting conveyor belt tracking, EE was using hammer to strike adjustable return roller bracket. EE missed hitting bracket causing his right arm to be drawn into belt & he became wedged between belt & roller.

Employee was assisting in replacing a bolt on primary jaw crusher. While striking a slugging wrench with a sledge hammer, employee's hand struct ajoining metal rail causing a fracture of the tip of right thumb. Employee didn't go to clinic until two days after accident.

WHILE REPLACING A SCREEN IN A SAND SCREEN DECK, SCREEN SLIPPED AND CAUSED LACERATION OF TWO FINGERS ON RIGHT HAND (SUTURES REQUIRED).

WHILE RE-ATTACHING A GUARD THAT HAD BECOME LOOSE, EE WAS CUTTING WIRE-TIE ATTACHMENT AND UTILITY KNIFE SLIPPED AND CUT EE LEFT THUMB

While cutting a conveyor belt, utility knife slipped cutting employee's right forearm.

ICY GROUND SURFACE THAT WAS PLOWED AND SANDED, EE SLIPPED ON UNEVEN SURFACE, FELL ON LUNCH PAIL INJURING RIBS.

LOADER SEAT ADJUSTMENT IMOPERABLE LEADING TO OPERATOR COMPLAINT OF UNSPECIFIC BACK PAIN. OPERATOR REFER TO PCP. ORDERED TO BED REST UNTIL 5-6-02. LOADER WAS REPLACED WITH LOANER UNTIL NEW SEAT ARRIVES.

STANDING ON THE FEED BOX OF A CRUSHER, 15' OFF THE GROUND, EE WAS REACHING ABOVE HIS HEAD TIGHTENING 2 BOLTS WITH A WRENCH. THE WRENCH SLIPPED OFF THE BOLT CAUSING EE TO FALL BACKWARD TO THE G ROUND.
