Employee was cleaning out a toolbox in the shop. A utility knife with the blade open was in the bottom which EE cut EE's finger on. EE was taken to Concentra walk-in clinic where they medically glued the cut closed. No impairment and no lost time.

Showing 50 of 102A subcontractor (Two Eight Drilling) was moving a drilling machine above the high wall with the drill in the air. The piece of equipment started to roll over. As it did, the subcontractor's operator, jumped out of the cab. In doing so, EE fractured EE's cheekbone, broke EE's arm, and had a laceration on the side of EE's head. All are non-life threatening.
Employee fell approximately 5 feet down from scaffolding while working on primary crusher. Employee has 2 fractured vertebrae and a hematoma in thigh, all of which are non-surgical injuries.
Employee changing drive belts on feeder motor. Come-along holding motor broke which resulted in the motor base dropping on employee's hand lacerating EE's little finger and possibly injuring other fingers.
Laying out future blast holes when rock and debris began falling from the above bench area.
Employee turned to walk away from skid steer after talking to operator. When skid steer started to move, left foot of employee was run over. Damage - severe bone bruising, 6 different fractures in EE's toes and crushed nerves. Employee was released on modified duty day of accident. Used crutches and a boot on the left foot.

EE was helping change hammers in the Stedman. EE had a pry bar wedging one hammer up to assist tow other EE to install the hammer into place. Hammer went into place but caused ee hand to be pinched between bar & hammer.
Hearing loss in left ear per copies of the report which indicated the injury for 3 different audiometric tests done on 2/10/15 and 3/16/16.
Hauling shot rock to Jaw (primary) - while returning to the truck from a restroom break, rolled right ankle while stepping on a rock, didn't see. Medical treatment resulted in modified work duties. Return to work pending follow-up with doctor.
Called into MSHA emergency line on 10/19/17. EE found unconscious - called 911 transported by ambulance to ER-released.
While shoveling at the secondary plant, the employees foot slipped from the shovel and when employees foot landed, employee felt a sharp pain in left lower back & buttocks area. Medical attention resulted in restricted work activity.
Employee states while shoveling rock they hurt their shoulder. Employee notified the supervisor on Friday 5/5/2017 that the injury occurred on 4/27/2017. Employee currently off work due to restricted activity.
While replacing and tightening a bolt on a plant employee felt/heard a pop in right shoulder. After tightening a bolt on a plant, employee started to feel pain and had difficulty moving arm. Medical attention resulted in restricted work activity.
Extension cord caught on truck was throwing off from truck, stepped off truck, landed on both feet, right foot tibia fracture.
Working at bottom of feeder pan, chip down to bare steel to find plug hole; burn out the weld to move the plate-rust & fine dust on plate-immediately tried to wipe from eye and flushed eye-trouble with eye on way home; awoke to bad pain at 1:30 am; 5:00 am eye swollen shut. Medical attention sought 1/26/2017.
Over Christmas weekend, between 12/23 and 12/27/2016, refuse pile gave way causing damage to drag line, box ben and #2 conveyor. No injuries occurred.
Injured employee was helping another employee get a screen loose on the pep screen. They pulled on the screen and when it came loose, injured employee's right hand was mashed between the lever and the pep screen. Injured employee remains off work pending further doctor evaluation.
While employee was doing clean-up work using a shovel, ee reported ee injured back. Medical attention at local hospital.
Loading holes, unrolling & moving the dewatering hose, hurt low back
While cleaning the front window on CAT 769c truck, stepped back and foot slipped, caught right forearm on the hand rail. Treated/Released from clinic.
While walking to the Stedman feed belt at secondary plant, EE slipped & fell at a muddy spot hitting right arm on a rock that had been removed from the feed belt. According to foreman, no trip hazard, just mud from all the rain. EE take to ER then transferred via ambulance to Hospital where her right elbow was reset. EE remains off work pending further evaluation
On Monday (9/28/15) employee slipped and hit right leg while cleaning retaining wall and tanks. Right leg continued to get worse, left work early on Thursday (10/1/15). Hospitalized 10/2/15 for infection. Remains off work.
While climbing down from the cab of a bulldozer, placed right foot on the step down bar, then lifted his left foot to step down onto ground when his right foot slipped between the step down and the tracks. He fell backward with his right leg caught straining right knee. Treated at ER, off work until further evaluation.
While replacing a wall hammer in the crusher the hammer slipped out of position. Injured person & another employee lowered the hammer to re-position their grip so the hammer did not fall onto a 3rd employee. IP did not remove his hand causing the hammer to sit down on his hand pinching his hand/fingers. X-rays revealed no fractures & he was released to return to work.
Employee was washing off concrete pad. The employee went to pull the water hose with his left arm when the hose got caught on something and jerked his arm back. The employee heard and felt a "Pop" in his elbow and had immediate pain. The employee was treated at the Emergency Room for his pain and was instructed to follow up with an Orthopedic doctor.
While turning hammers over in the Stedman crusher and being assisted by another EE the other EE's end slipped from his hand crushing injured employee left ring finger. His finger was caught between hammer and the plated it attaches to causing laceration and contusion. First aid was performed at plant. ER treated and released day of injury. X-ray at ER showed not broken bones.
While lacing the lime screen belt back together with electric drill, the drill twisted causing his hand to be torqued before he could let go of the drill. Thought was a sprain, but next morning hand & finger swollen. Medical treatment 7/16/15 revealed broken right ring finger. Company accommodating restricted work activity.
While in transit on a company errand, truck employee was riding in came to a stopped position and was rear ended. Employee was treated and released at ER.
While replacing the skirt rubber at pioneer #3, noticed the hole was not correct and as employee was re-cutting the hole, his knife blade slipped cutting the top joint of his left thumb. First aide performed at facility. ER treated and released day of injury.
While backing CAT truck into position to be loaded, employee failed to notice the loader bucket was not high enough thus causing contact between both CAT truck & loader while still in backup motion. Resulting in a whiplash type motion causing pain in his neck and a headache. Minor medical treatment on 11/13/14.
Employee was holding jaw bolt & pin while the pin was being hammered out; thought his hand was clear of pinch point but as the pin came free, his hand slipped behind pin & wall pinching his right ring finger. Medical attention on 9/3/14, Restricted work activity.
Employee finished grinding metal, while removing safety glasses, felt something enter his right eye. He used wash solution at the 1st aid station and thought ok. Overnight his eye became irritated, sought medical attention on 4/8/14. Doctor treated and released.
Turning primary impact pulley, the index, middle and ring fingers of left hand smashed between v-belt and pulley. Fracture middle finger, contusion, abrasion index and ring fingers. Medical treatment. Restricted work activity.
End of shift, washing out skid-steer bucket and pins; when employee pulled out pin the bucket fell on his left foot. Fracture left big toe bone. Employee remains off work
Employee airing up tire on wheel barrow, put too much air in the tire exploded. Tire parts hit employee on side of head. Treated and released from ER.
While hauling a load of rock off the mountain to the crusher; employee stated the truck started bouncing in the front end. The truck hit two boulders, knocking them off the high wall. Impact caused the steering wheel to jerk the employee's left hand resulting in injury to wrist on the left side. Medical treatment, restricted work activity.
Employee operating CAT truck, moving shot rock from lower level of quarry to upper field for stockpile. Cat truck went over the wall into lower level.
While working in the air separator, operating a hand grinder to grind down the welds on the sides and wearing safety goggles and a full face shield, employee got something in his right eye. Medical Treatment on 3/7/13 with follow-up scheduled.
While employee was tightening up jaw die bolts at big jaw crusher #521013; his feet slipped on some grease causing him to over extend himself with his right leg; resulting in injury to groin area. Medical treatment on 6/29/12; modified work for 3 days
EE was on the drive side on the tunnel conveyor hooking up a comealong to pick up bottom of the belt off old wooden crosstie. New wooden c.t.'s being pushed in place across the metal frame to the drive side of conv., the EE reached down to grab c.t. and mashed finger tip between wood and conveyor metal. ER 2/22/12 Surgery 2/23/12, Rt. middle finger tip. Remain off work.
Loading a piece of steel in the loader bucket; the piece of steel slid and mashed finger between piece of steel and the cutting edge of the loader bucket. Medical treatment on 1/31/12: amputation Rt 4th finger tip: 2/1/12 return to modified work, unable to use rt hand.
EE was putting belts back on primary impact: mashed nail of left ring finger between V belt and thieve on impact motor. 2/13/11 sought medical attention at ER. 2/14/11 sought medical attention at ER. 2/14/11 return to modified work, Rt hand only. Note - 2/14/11 - 2/18/11 schedule vacation taken.
EE was on the discharge belt to get a wedge off of it, when the plant operator did not see him and started up the belt. EE's left hand got caught between the roller and conveyor belt. EE was treated and released from the ER with bruising.
While repairing a conveyor belt, EE was cutting a hole in the belt using a utility knife; the knife slipped and severely cut EE's left hand. EE returned to restricted work activity on 7/29/10.
Employee was changing filter on 769C Cat truck. He bent down under truck to use impact wrench. He bent over to reach filter and felt something pop in the middle of his back.
Employee was welding in a truck bed and was chipping the slag when same entered left eye.
Employee was moving rocks out from under jaw. One rock was too big to move so he was breaking rock with a sledge hammer when a piece of the rock hit his leg causing swelling to the right leg. After receiving medical attention, was released to regular work on the day of the injury. Did not report to work Friday 6/6/08, stating due to injured leg.
Employee was jacking trailer off the ball-hitch was stuck on ball-when hitch release it smashed right index finger between the hitch on the trailer and the military hitch on truck. He worked 5/2/08 and is currently off work, effective 5/5/08. Note: currently employee tn shop-delivery to huntsville al quarry.
EE was pulling crusher, stepped thru hole in cat walk, landed on right knee. Cut/scraped right knee. Currently working under restricted work activity. Note: currently employed in welding shop-working at Smyrna Quarry day of injury.
